We could tell you again how valuable it is to be part of HCCI’s Illinois House Call Project (ILHCP), but instead, let’s hear what two participants from the first cohort have to say:

Passionate Illinois providers — including independent practices, those affiliated with health systems, and hospice and palliative care programs looking to add or expand their home-based primary care services — can apply for the second cohort by Friday, May 3.
Selected practices will receive over $25,000 in benefits, including a full practice “checkup,” comprehensive training and resources, the opportunity to acquire capacity-building funding, and exclusive access to industry experts.
