Passionate Illinois providers — including independent practices, those affiliated with health systems, and hospice and palliative care programs looking to add or expand their home-based primary care services — can apply for the second cohort by Friday, May 3.
Selected practices will receive over $25,000 in benefits, including a full practice “checkup,” comprehensive training and resources, the opportunity to acquire capacity-building funding, and exclusive access to industry experts.
