Dear Home-Based Primary Care Practice Colleagues,
The National Home-Based Primary Care Learning Network is now accepting applications for the eighth wave of its Learning Network, comprised of 81 practices across 36 states.
The National Home-Based Primary Care Learning Network, funded by The John A. Hartford Foundation, has led efforts over the past few years to improve care delivery in home-based primary care (HBPC) by creating a national learning community of HBPC practices focused on care quality and quality improvement.
Please see the Request for Application for additional details on the HBPC Learning Network and application process. The submission deadline is May 15.
