Apply for New Award to Receive CCM Implementation Services

Community-based palliative care programs in Illinois can apply for the new HCCI & Elea Institute Innovation Award program to receive support in implementing Chronic Care Management (CCM) services to expand and diversify their offerings and help drive revenue.

 

HCCI will select up to 10 community-based palliative care programs in Boone, Cook, DeKalb, DuPage, Kane, Kendall, Lake, LaSalle, Lee, McHenry, Ogle, Will, and Winnebago counties to participate, beginning in October. Award recipients will receive a fully subsidized CCM Implementation Package.

House Calls Program Achieves Over $760K in Savings in Six Months

Using Home-Based Primary Care to Improve Quality of Care & The Bottom Line

Based on a six-month study, Rush University Medical Center in Chicago achieved the following with its Rush@Home house call program in partnership with HCCI:

 

• 34% potential lower cost

• 18% fewer hospitalizations

• 21% reduction in ED visits

• 4.6% fewer readmissions

 

Total Program Savings

• Total Actual Six-Month Savings: $761,000

• Total Potential Annualized Savings: Over $1.5 million

 

Based on these results, a program with 200 MSSP patients would generate over $3 million in savings.
