Funding Priorities:


Within the following priorities, we are especially interested in supporting small and/or rural independent practices, as well as projects that focus on improving health outcomes for populations facing the greatest health disparities and inequities:


  • Provider-Led Health Care Innovation: Goal to implement intervention(s) specific to health care delivery systems, clinicians, and/or patients that focus on the quadruple aim of improved quality, reduced cost, improved patient experience, and improved provider and staff experience in the communities that COHQA serves.


  • Community-Led Health Care Innovation: Innovations to improve community health or patient and provider experience can and should come from the community itself. We encourage community members, as consumers of healthcare and owners of their own health, to partner with healthcare providers to develop and implement fresh ideas to improve care delivery.


  • Clinical Integration and Multi-Sector Coordination: Implementation of intervention(s) that advance clinical and financial coordination and integration of stakeholders in the communities we serve.


Eligible Applicants: Applicants in the counties of Crook, Deschutes, Grant, Harney, Jefferson, Lake Wheeler, Wasco, Sherman, Hood River, Skamania, and Klickitat: providers, clinics, or community organizations involved in health care improvement or community support. We encourage multi-stakeholder partnerships and collaborations.

History of the COHQA Grant Program

In 2013, COIPA started the grant program to fund innovative healthcare improvement projects within our network of providers and clinics. In 2016, COIPA supported the creation of the Central Oregon Health Quality Alliance (COHQA) to house the grant program and expand supporting projects and initiatives beyond the COIPA network. Since then, COIPA continues to be a significant contributor to COHQA’s work of supporting healthcare innovation from idea to adoption.


Over the last 10 years, COHQA has impacted the quality of healthcare across a range of specialties, issues, and patient demographics across our 40,000 square mile service area from North Klamath up through the Gorge and into Eastern Oregon. Through our partnerships with healthcare providers, public health, clinics, private and community foundations, and nonprofits, we have supported and invested $1.6 million in over 60 projects that are improving healthcare in this region.
