Tax Form Information

One of the most common questions we get this season is "how do I find my 1095 form for health insurance?"

To obtain that form, you will need to contact your insurance company directly. They do make it available online and may also mail it to you. You can log in to your member portal with your insurance company to download it, or call them using the number on the back of your ID card. Healthcare.gov will also have it available.  To access it via Healthcare.gov, please follow these instructions.
