Community Nurse Supports Thoughtful Health Care Planning

Advance Care Planning Made Simple

National Healthcare Decisions Day (NHDD), observed annually on April 16, is a nationwide initiative that encourages adults of all ages to reflect on and communicate their wishes for future medical care. NHDD helps normalize conversations about advance directives and end-of-life preferences so that patients’ values and goals can guide care if they become unable to speak for themselves.

Community Nurse supports patients and families through palliative care consultations with a Nurse Practitioner (NP) which helps them understand disease progression, explore care options, and clarify goals. The Nurse Practitioner also provides guidance on advance care planning, assisting with the Medical Orders for Life-Sustaining Treatment (MOLST) form, which documents medical treatment preferences. Another important topic discussed is the importance of choosing and documenting a Health Care Proxy to ensure someone they trust can speak on their behalf when needed.

To learn more about these and other important directives, check out Episode 4 of Your Care. Your Choice., a podcast by Community Nurse, featuring Atty. Rebecca Spinner, Senior Associate Attorney at Surprenant, Beneski & Nunes, P.C.

Frequently Asked Questions

What Is The Difference Between Physical Therapy and Occupational Therapy?

Physical therapy (PT) and occupational therapy (OT) both help people recover and live better lives, but they do this in different ways.

  • Physical therapy helps people move better by improving strength, balance, and walking. This makes it easier and safer to get around and stay active.

  • Occupational therapy helps people do everyday tasks like getting dressed, bathing, cooking, or working by building skills or changing how a task is done.

Together, PT and OT support not just better movement, but fuller, more independent living.

At Community Nurse, our licensed physical and occupational therapists provide this care in the comfort of a patient’s home. Working closely with patients, families, and other clinicians, we help individuals recover safely, maintain independence, and continue living well at home.

Meet Kathy Gauthier

In honor of National Administrative Assistant Day, we’re celebrating Kathy, a vital part of the Community Nurse team. She is one of the friendly voices and caring hearts behind Community Nurse. As our receptionist, she is often the first person our patients, families, and staff connect with, always greeting everyone with kindness, patience, and a smile.

From helping callers find the right person, to keeping communication flowing smoothly, Kathy’s dedication and compassion make a difference every day. Beyond her role at the front desk, Kathy is always ready to lend a helping hand, supporting community food and toy drives and bringing compassion to everything she does. We are so grateful for all she does and proud to recognize her as part of the Community Nurse family.
