We are very pleased to keep you well-connected with what’s happening at Cornerstone VNA. I enjoy seeing so many of you in the community and hearing your words of gratitude for the care you have received from Cornerstone VNA.

We are offering a number of community educational events this spring and I encourage you to take advantage of these if you can.

Important Dates and Events
Caregiver Cafés
Cornerstone VNA launched a new Caregiver Café in Newington this month at the Langdon Public Library, 328 Nimble Hill Road. The Caf é will be held on the l ast Thursday of every month at 10:30 am and is a time for caregivers to gather for support and education. Grief Support Group
Cornerstone VNA is hosting an 8-week Grief Support Group for adults on Fridays from 10am-12pm beginning April 6 . The goal of the group is to help individuals cope with feelings of grief, loss and sadness in a safe and caring environment. Details here .
National Healthcare Decisions Day
National Healthcare Decisions Day (NHDD) is April 16 . In honor of NHDD, people are encouraged to have a frank and open discussion with their loved ones and their healthcare providers about their end of life preferences and to make their wishes known.   Details here .
Caregivers Connect Event
Caregivers Connect is an afternoon event featuring education and support for family caregivers. This annual event will be held May 10 from 2:00 pm - 4:30 pm at the Comfort Inn & Suites Meeting Room in Dover, NH. Details here.

Falls Prevention Day
Falls Prevention Day is a special event for people at risk for falls. It will take place on May 23 from 9:00 am - 12:00 pm at St. John's Church in Dover, NH. Find details here .

This event is hosted by The Falls Prevention Task Force of Strafford County.

New Medicare Cards Being Issued
Starting April 1, 2018, Medicare will begin issuing new Medicare cards. The new cards are designed to prevent fraud and fight identity theft. Find complete details here .

Featured "Rock Star" Team Member
Our Rock Star program celebrates staff who are "caught caring" by a coworker, or recognized for exceptional care by patients and their families. This month's rock star is Erica Heon, RN, who was recognized by our CEO, Julie Reynolds, for her work as a preceptor.

According to Erica, "Being a home care nurse allows me to create a plan of care that is unique to each patient. I believe the best healing happens at home, and it's wonderful to be a part of that process - from day 1 to discharge."

Did you know?
Did you know that March was National Social Worker Month? And did you know that Cornerstone VNA has medical social workers on staff who work with patients? Meet Anna, Julie, Sharon and Hollie (pictured).

Medical Social Workers are part of our multi-disciplinary team and work closely with our nurses and therapists to meet the needs of our patients and their families. Specifically, the Medical Social Workers address the social, economic and emotional issues that an individual may experience related to their illness or injury. Medical Social Workers offer the following services:
  • Assessments of social, economic and emotional needs
  • Education and access to community resources
  • Long term care planning
  • Assistance completing advance directives
  • Counseling and education to improve coping and reduce stress
