Regulatory Tip of the Week: Specimen Labeling

Per The Joint Commission National Patient Safety Goal 01.01.01: Use at least two patient identifiers when providing care, treatment, and services, and label containers used for blood and other specimens in the presence of the patient.

Mislabeled specimens can cause patient harm by delaying treatment or by inappropriately administering treatment based on incorrectly assigned results. To ensure quality and accuracy in our testing we are required to reliably identify the patient as the person for whom the service is intended and to match the service to that patient.

CALL TO ACTION:

  1. Always verify patient identity using 2 qualifiers: a.) Patient first and last name, b.) Date of birth, c.) Medical record number
  2. Never label tube/specimen container prior to obtaining specimen.
  3. Never leave the room/bedside before labeling the tube/specimen container.
  4. Never collect specimens from a patient without an ID band.
  5. Never double label a specimen/container.
  6. Never place label on or over lid of specimen/container.
  7. Always place labels on specimen/container in a readable fashion.
