1. Details for Form 1095:
a. All consumers received Form 1095 in their secure inbox in the platform. There was no option to opt out of receiving Form 1095 electronically.
b. All consumers received a Form 1095 by mail unless the specifically requested not to on the Communications Preferences Page.
i. This is a distinct paperless indicator on the Communications Preference Page. While consumers may have opted out of other paper communications, they will still receive a paper Form 1095 by mail unless they choose to opt out of it.
ii. The address used in the address block on the Form 1095 is the address the consumer used for their communications preferences and may be different than their home address.
iii. The address printed on the Form 1095 is the home address and the same address used when the plan was quoted.
c. Consumers received a Form 1095 for each plan they were enrolled in during PY2024.
d. If an enrollee is filing taxes separately from the rest of the household, the enrollee will receive a separate Form 1095.
e. Only consumers who have enrolled in a QHP will receive or be included on the Form 1095.
f. Consumers enrolled in catastrophic plans or stand-alone dental plans will not receive a Form 1095.
g. If you have a consumer who did not receive a Form 1095, please put in a ticket.
