All providers who have received CARES Relief Payments (HHS funds) are required to:
- Utilize the HHS portal to complete attestations within 90 days of receipt of EACH payment
- Confirm receipt of payment, agree to the terms and conditions and provide additional information as required with each attestation
- Maintain compliance with the terms and conditions for each attestation
- Maintain appropriate documentation to support each attestation
All providers who received more than $150,000 in total COVID-19 relief acts (including PPP) are also required to submit reports 10 days after the end of each calendar quarter. This is anticipated to start for the calendar quarter ending June 30.
