Comprehensive Case Management for Adult Medical Day Services



Empowering Minds Resource Center proudly provides Comprehensive Case Management for participants in the Maryland Medical Day Care Services Waiver (MDCSW). Our dedicated Case Managers help individuals and their families navigate Medicaid and community-based resources, ensuring access to the services needed to live safely and independently.


What is Comprehensive Case Management?


Comprehensive Case Management is a free service available through the Maryland Medical Day Care Services Waiver. Your Case Manager serves as your advocate and guide—helping you understand your options, coordinate care, and connect with the resources that best support your health, independence, and overall well-being.

Our services are completely separate from Adult Medical Day Care centers, providing conflict-free, participant-directed support focused on your individual goals.


Your Case Manager Will:


  • Get to know your goals, strengths, preferences, and support needs.
  • Coordinate services such as Adult Medical Day Care, medical and behavioral health care, physical, occupational, and speech therapy, transportation, durable medical equipment (DME), and community resources.
  • Monitor your services to ensure they continue meeting your needs.
  • Identify and address concerns, risks, or changes in your care.
  • Manage and report required reportable events while advocating on your behalf.

What You Can Expect


  • Regular phone and in-person check-ins.
  • Required face-to-face visits at least every 180 days.
  • Timely support whenever your needs or circumstances change.
  • Ongoing communication to help you stay connected to the services you need.

PRP for Adults


Our PRP program for adults focuses on structured group activities that help develop and strengthen independent living skills. The program supports individuals in successfully reintegrating into the community while maintaining and enhancing skills for long-term success.


PRP for Minors


The PRP program for minors is person-centered, supporting positive growth in behavior, social skills, and family dynamics. By empowering youth to achieve their personal goals and objectives, the program helps foster healthy development and lasting progress.
