Public Safety Officers’ Benefits Program
A unique effort of the U.S. Department of Justice; local, state, tribal, and federal public safety agencies; and national organizations, the Public Safety Officers' Benefits (PSOB) Program provides death and education benefits to survivors of fallen law enforcement officers, firefighters, and other first responders, and disability benefits to officers catastrophically injured in the line of duty.
The Bureau of Justice Assistance PSOB Office is honored to review the more than 1,200 claims submitted each year on behalf of America's fallen and catastrophically disabled public safety heroes and their loved ones.

Please ONLY complete ONE Tobacco Affidavit and Wellness Form.
Please take the time to complete the Tobacco Affidavit/Wellness Incentive form on E-Forms before October 24, 2025.
Review all options of the tobacco affidavit before submitting. You will only have one chance to submit the form. If you select option 2 on the tobacco affidavit, you are required to attach the tobacco certificate of completion to submit the form. (the certificate must be attached before October 24,2025, to avoid the tobacco surcharge for 2026). There will be no need to contact Human Resources when you have completed the tobacco session classes. We will receive a list of the participants who completed the tobacco cessation program. Any tobacco surcharges incurred in 2025 will be REFUNDED in the month of December 2025.
If you are unfamiliar with Neogov, I have attached instructions to complete the forms and to complete the tobacco cessation sessions. If you do not have a username/password or forgot your username/password, please contact the Human Resources Employment Team (Kelly Denham, Raven Williams, George Marshbanks) at 706-653-4059 to assist.
*Note: ONLY employees enrolled in CCG’s medical plan or will enroll in the medical plan in 2025 should complete the tobacco affidavit/wellness incentive forms.
Please DO NOT inter-office, email or fax the Tobacco Affidavit or Wellness Incentive forms to Human Resources. Both forms should be submitted on E-Forms at https://eforms.neogov.com/self-service/portal .
Remember, if you are on FMLA, vacation or administrative leave etc., the tobacco affidavit should be completed by October 24,2025, to avoid the tobacco surcharges in 2026. No late forms will be accepted.

Allow 15 minutes after initial sync for your data to appear.

Get Help & Support
If you’re in emotional distress or suicidal crisis, find help in your area.
If it’s an emergency, call 911. You can also go to your nearest emergency room. A medical professional can give information about your particular circumstances.
If you believe that someone else is in danger of suicide and you have their contact information, contact your local law enforcement for immediate help. You can also encourage the person to contact suicide prevention hotline at 988 (call or text).
When you’re in crisis, research suggests that it may be helpful to reach out to others. When you connect with others, it can help you feel supported and give you a sense of belonging.
