PACE of Northeast Indiana

Programs of All-Inclusive Care for the Elderly (PACE) is a Medicare and Medicaid program that helps people meet their health care needs in the community instead of going to a nursing home or other care facility. With PACE, you have a team of health care professionals, the Interdisciplinary Team (IDTeam), working with you and your family to make sure you receive the coordinated care you need.

PACE provides all the care and services covered by Medicare and Medicaid, as well as additional care and services deemed necessary by the IDTeam.


This includes coverage for primary and specialty care, prescription drugs, medical transportation, hospital visits, dental care, medical equipment, home care, as well as physical, occupational, and speech therapy, and other services. The IDTeam works together with you and your family to develop the most effective plan of care for you.

PACE Eligibility

There are four criteria you must meet to be eligible for the PACE program. First, you must be 55 years of age or older.

You must also live in Allen County, the PACE service area.

PACE Participants must also qualify for nursing home level of care. This means that they require assistance with activities of daily living, such as bathing and getting dressed.

Lastly, Participants must be able to safely live in the community with the help of PACE services. 

PACE Enrollment

Enrolling in PACE of Northeast Indiana is an easy 4-step process:

First, a member of the PACE enrollment team will visit you in your home and conduct any necessary assessments.


Next, if you haven't done so already - you'll visit our PACE center to meet the IDTeam. 


Next, the IDTeam will meet and create your unique care plan. This plan is then presented to you and your caregivers for you to confirm whether it meets your needs. 


Finally, the state of Indiana confirms your eligibility and you sign your care plan. Your services will start on the first day of the following month.
