Hello All,
Thank you for the hard work and resilience you have shown all year to provide outstanding service to care for our patients during trying times amid staffing challenges while navigating your own personal stressors. You met the challenges of the last year with resilience, grit, and a tireless dedication to service. I am grateful for all that you did to help us fulfill Baystate Health’s mission of improving the health of those in our communities with quality and compassion.
Your contributions inspire me and remind me of one of my favorite quotes: “No one can do everything, but everyone can do something.” We could not have gotten through the year without your talents and your commitment to our mission. At each step of the way, we did not lose focus. WE took care of our patients, and WE took care of one another. Our teams confronted this year’s challenges with bravery, patience, and a truly collaborative mindset. I thank you all very much for that.
Your willingness to always go above and beyond is what makes Baystate Health the amazing place that it is. I feel proud and honored to be able to work with you all towards a common goal. The year ahead will surely bring challenges, but I am confident that we are prepared to take them on together. I look forward to continuing working with and learning from each of you in the new year.

My holiday message to the team is one of gratitude and hope. I am forever grateful to all of the individuals that work for the laboratories of BRL. Your hard work and perseverance through a challenging few years is remarkable and to be applauded. You have met every challenge with the sprit of enthusiasm and unending support for the patients of Baystate Health and each other. My sincere hope is that you are able to enjoy some much needed time away from work with your families and friends to recharge your batteries and reflect on all that you have for which to be grateful.
On a personal level, I need to thank the team of individuals who have supported me through a challenging year of health concerns in my own family. I consider myself so lucky to be surrounded with a team of people who really care about each other and that I consider lifelong friends.
I look forward to the promise of a new year, 2023 brings the opportunity in so many ways for continued healing, successes and friendship.

Dear All,
As we navigate the contemporary times of great resignations, quiet quitting in general workforce and layoffs mostly in the IT services sectors across the country, we are grateful to each other for continuously serving the profession of laboratory medicine. The year 2022 for the Clinical Chemistry and Point of Care Testing service lines within Department of Pathology have been of a mixed bag of successes and opportunities for improvements. I am thankful to the team for successful implementation of High Sensitivity TnT across the Baystate Health without any significant glitch, and I want to applaud the Point of Care Testing team for their diligent efforts in having the Proficiency Testing compliance back in track for the PPMP (Provide-Performed Microscopy Procedures) services.
I am looking forward to the renewed efforts around the challenges we face in the clinical laboratory of BRL. We must adapt to the new thinking to find solutions for the problems and challenges we face in the lab on day to day basis.
Albert Einstein once said ‘We can't solve problems by using the same kind of thinking we used when we created them’. He also said elsewhere ‘If I had an hour to solve a problem I'd spend 55 minutes thinking about the problem and five minutes thinking about solutions.’
We are a high volume clinical laboratory with immense potential to add value to the Baystate Health patient care and the population of Western Massachusetts by bringing more business viable esoteric tests in house. However, we do need to face the financial challenges of the day by renewed thinking and spend more time on 5Ws of what, who, when, how, and why a problem is to be solved. This can significantly improve the focus and effectiveness to achieve high value patient care through accurate and timely diagnosis.
My new year resolution of 2023 around BRL is to continuously improve Clinical Chemistry services and spend substantial time (55/60 minutes!) on planning to set up an Advanced Special Chemistry Laboratory within BRL. That would require a fresh thinking, even undo some of the previous thinking. I am counting on the Chemistry Team, Pathology, BRL and Baystate Health Leadership for continued support while I keep working on the 5Ws of an Advanced Special Chemistry Laboratory.
With this resolve, I wish you and your family a very happy and blessed holiday season ahead. I wish that the new year of 2023 brings all the bounties around your and your loved one’s life.

Please join me in Congratulating Justin Rogers for his promotion as the new BMC Chemistry Laboratory Supervisor
Justin came to BMC from Ann Arbor Michigan Medicine. While at Michigan Medicine from 2016-2020, Justin held several roles and assumed different responsibilities as one of the Senior Technologists including overseeing the lab in the Special Chemistry area. He also helped in the IT Test build in the Molecular area for SOFT LIS.
Before his role at Michigan Medicine, Justin worked at Yale-New Haven Hospital (YNHH) from 2008-2016. At YNHH, he started working as a night shift Medical Technologist in the General Chemistry lab and quickly progressed to a Lead Technologist role on the day shift in a few years time. As a Lead Technologist, he worked in the Special Chemistry section. He performed specialized testing such as Mass Spectrometry, LCMS and Bio Disease Detection lab testing. He was involved in the development of some testing methodology such as Ethylene Glycol on Teledyne and Agilent Headspace analyzers for GC and Methylmalonic Acid and Acylcarnitine for LCMS. He also developed Iron Key interface for LCMS. He has been a guest lecturer at the University of Bridgeport sharing his expertise on the subjects of LCMS and GCMS. He also rotated in the Automation area of the General Chemistry Lab as one of the senior techs providing guidance to the team. He is one of the subject matter experts (SMEs) during the development of YNHH's EPIC Beaker laboratory information system.
Here at BMC, Justin started on 2nd shift as a Medical Laboratory Scientist I in November of 2020. He later transferred to day shift and very recently was promoted to Chemistry Automation Specialist and later moved on to become the Supervisor in the Chemistry Laboratory. He has made process improvements and efficiencies in the Automated area and is continually working with staff and Leadership in moving the department towards a better environment and organized workflow.

Please join me in Congratulating Daniela Kotorabay for her promotion as the new Chemistry Automation Specialist
Daniela began her career as an intern at The Walter Reed Army Institute of Research in UMASS. She spent her junior and senior time there exploring and learning about research and finding out if that is the right path for her career.
After receiving her degree in Biology, she decided research is not what she wants to pursue. She then found the Medical Laboratory Sciences program at STCC where she enrolled herself to learn more about the Laboratory profession.
While she was completing her clinical rotation at Holyoke Medical Center, she also worked as a Processor. She then took a job at Baystate Medical Center as a Medical Laboratory Technician on 2nd shift. While working at BMC, Daniela had multiple interactions with me, and during our meetings, I discovered her passion to make things better and improve processes that will eliminate inefficiencies in the workflow. With her interest to learn more, she transferred to Noble and work there as a Generalist. While at Noble, she obtained her ASCP Certification as a Technologist in Chemistry. Recently, Daniela came back to the Chemistry Lab working on dayshift at BMC and was promoted to become the Chemistry Automation Specialist.

Please welcome Melissa Nunez, the new Lead PSC Phlebotomy Technician
Melissa has been with Baystate for 6 years. She has routinely taken charge responsibility on weekday evenings and weekends. Staff from different PSCs contact her for questions related to Sunquest and Doctor’s Orders, as she knows how to troubleshoot and resolve problems.
Melisa will be working from 8:30-5 at 100 Wason, while also assisting 140 High St, 222 Carew, Brightwood, 3640 Main, and 50 Wason. She has had many outstanding accomplishments this year including becoming an expert in Atlas and the ordering and handling of IVF/ transplant specimens. She has assisted with the training of new employees and students. She has worked in various Patient Service Centers as needed and demonstrates great flexibility with a positive attitude.
Melissa is very excited about her new role and looks forward to overcoming future challenges. We are very happy to welcome her onto the Lead team!

We would like to congratulate Tammy Woodruff on her promotion to Lead Phlebotomist in October
Tammy has been with BRL for fourteen years, started as a mobile phlebotomist and went back and forth a couple times to permanent positions.
She has been involved with a few A3 projects. Tammy traveled east to West Brookfield covering that site for a month. She worked for about three years on Saturdays at Indian Orchard. Before moving to Boston Rd, Tammy was in Belchertown at Swift River.
She enjoys her work and tries to make the patients' experience the best in her care. Tammy was involved with the United Way for a couple of years. In her spare time. Tammy enjoys cooking, travel, and yoga.

Implementation of the 5th Gen High Sensitivity Troponin T at Baystate Health

On November 15, 2022, the clinical chemistry laboratories across Baystate Health - Baystate Medical Center, Baystate Franklin Medical Center, Baystate Wing Hospital and Baystate Noble Hospital - went Live with the 5th Generation High Sensitivity Troponin T (hs-TnT).
The High Sensitivity Troponin T Workgroup - a multidisciplinary team comprised of members from Emergency Department, Cardiology, Hospital Medicine, Clinical Informatics and Pathology developed consensus algorithms to implement and utilize the 5th Generation High Sensitivity Troponin T based on the recommendations of Fourth Universal Definition of Myocardial Infarction (2018).
Successful implementation of hs-TnT at Baystate Health would help reduce time to rule out MI with better accuracy and reduce wait time in the ED. This should cumulatively result in admitting lesser number of patients with high confidence ruling out MI presenting in ED with chest pain and suspected acute coronary syndrome. This will also help reduce costs in terms of laboratory expenses by reducing the number of current generation Troponin T tests ordered to rule out MI.

Sysmex XN-9100 Go-Live at BMC and Systemwide Lab Implementation of Caresphere Workflow Solution Middleware

On October 12, 2022, after months of planning, BMC Core Hematology launched a successful Go Live for the new and improved XN-9100 line. This system consists of four individual cell counters (XN-10), two individual slidemakers/stainers (SP-50), two individual cell imager for WBC differrentials and RBC morphology (DI-60), and a tube sorter (TS-10).The XN-10 CBC modules are designed to provide accurate CBC results with comprehensive flagging and rerun/reflex rules for reliable whole blood screening. The XN-10 analyzer offers CBC, automated differential, fluorescent platelet, reticulocyte, and body fluid testing. The XN-Series modules and track components are integrated to increase testing capacity (additional throughput of up to 100 CBCs/analyzer/hour). On the same day, we assisted with a BRL wide middleware upgrade to Caresphere Workflow Solutions (CWS). CWS is used for management of and decision making on patient results from the Sysmex XN analyzers - and is utilized here at BMC as well as the Community Hospitals. Upgrading the hardware at BMC and the middleware across Baystate Labs has allowed us to streamline the workflow and provide faster turnaround times.
We are thrilled to have these technological advances in our laboratories that will aid in providing a higher level of quality patient care. A big thank you to all of those who were involved in the process, including Dr. Johari, Darlene Cloutier, Fernando Carlos, the LIS team, BMC Core Hematology, Central Processing, BWH, BNH and BFMC. We are also incredibly appreciative of Juan Rodriguez, Sysmex Service Engineer III, and his team at Sysmex. We experienced many trials and tribulations, but I am so proud of the team for overcoming the hardships and coming together to make this project successful.

Lara Bonomi, Supervisor of the Molecular Pathology Lab at BRL, presented her teams' results on using tertiary analysis software in evaluating DNA sequencing data from myeloid malignancies at the 2022 Annual Meeting of the Association for Molecular Pathology (AMP) in Phoenix, Arizona in November 2022.
After acceptance of her abstract, Baystate Health cancelled all corporate travel due to budgetary concerns. Lara then applied for and was awarded an AMP Technologist Travel Award that allowed her to attend the meeting and present her work.
Congratulations to Lara and her team for their dedication and commitment to the patients served by BRL!

TRANSFUSION MEDICINE SERVICES (TMS)
BMC Transfusion Medicine and Blood Donor Services Received AABB Reaccreditation

BMC Transfusion Medicine and Blood Donor Services received reaccreditation by the AABB (Association for the Advancement of Blood & Biotherapies formerly known as the American Association of Blood Banks) following a successful 2-day inspection on October 12-13, 2022.
Routine and ongoing assessments of transfusion service processes help ensure the maintenance of a robust program focused on patient outcomes and safety. Quality assessments can help to prevent mistransfusions and several other noninfectious serious hazards of transfusion. Diligent monitoring, identification, reporting, and rebuilding of problematic systems that put patients at risk can help to achieve this goal. AABB accredited institutions maintain a quality plan and are evaluated by a complete review of each transfusion process including:
· Procedures
· Staff education and training
· Blood component ordering
· Patient consent
· Sample collection and compatibility testing
· Blood administration and transfusion documentation
· Adverse events and transfusion reactions
· Blood utilization
· Incident and deviation management
The assessor placed a heavy emphasis on direct observation of staff and was impressed by everyone’s knowledge and ability to articulate the “why” behind the actual work being performed.
Congratulations to the TMS and BDP Teams for a successful assessment with no nonconformances – well done!

Students from Baystate/Springfield Educational Partnership (BSEP) Program Visited BMC and Whitney Labs
This program helps to advance education for high school students and prepare them for careers that serve the health and wellbeing of the community. The programs use hospital-based learning experiences to provide students with career options, comprehensive observation experiences and prepare for employment opportunities.
BRL's BMC and Whitney labs hosted a BSEP career exploration group that was interested in laboratory careers. Staff donated their time on Halloween night to share their passion about their career choice. The BSEP group was very engaged and appreciated everyone’s time.

The Laboratory Workforce Development Team is attempting to start a lab lecture series centered on answering staff questions or providing more in-depth information on topics of interest. Please hang the attached flier up in your respective areas and share information about this initiative at staff meetings/huddles. Participation at all levels is key to its success, so please consider sharing an idea or volunteering to present. We look forward to hearing everyone's ideas!

This month I would like to focus on slips, trips, and falls. Wherever these events occur, inside or outside, most are preventable. As the weather turns cold and the snow starts to fly, it is a good idea to keep boots and sand in your car. This way you are always prepared for a weather change.
Tips to prevent outdoor slips, trips, and falls:
-park in a cleared area
-wear boots or rubber soled shoes
-plan the route you need to take
-avoid carrying items
-use your vehicle for support until you are sure of the conditions
-test before your step
-report unsafe conditions
-wipe your feet when you enter a building
Tips to prevent lab slips, trips, and falls:
-watch for wet floors
-look out for walking surface irregularities
-report inadequate lighting
-prevent clutter
-keep items off the floor - boxes, wires, cords, etc.
-close doors/drawers
-don’t rush
-use stepstools
-keep door ways and aisles clear
All incidents/accidents are evaluated for trends which is why reporting all incidents/accidents is so important. If we find a trend and implement a safety measure, we can prevent future issues.
