Welcome back to our wonderful school families and a huge warm welcome to all of our new families! We are so thankful you all are here and look forward to a wonderful school year at St.Agnes Academy-St.Dominic School.
Over the summer, I hope you all made some beautiful memories, got a little rest, and are now ready for a wonderful school year! As we begin the 2022-23 school year, I wanted to reach out with some important information that will aid me in the clinic when caring for your child.
Immunization Forms:
Please send in an updated copy of your child's immunization record at the start of every school year. Please remember this needs to be on a Tennessee Department of Health Certificate of Immunization, regardless of what state you reside in. Please email updated copies to [email protected] or send a copy in with your child.
Health Plan -When to Stay Home:
Please review the school Health Guidelines on general illness. If students are showing any signs of illness, please keep them home and email the school nurse at [email protected], your child’s teacher, and your division’s administrative assistant.
Medications/Medical Forms/Emergency Medical Form:
If your child has an illness that requires medication, whether it’s daily or for a short period of time, the appropriate paperwork must be completed in order for the medication to be administered. Over the counter medications will only be administered to students whose guardians have provided consent on the emergency medical form or by verbal consent. Please do not send in medications without the appropriate form and an email to the nurse.
