St. Luke School Basketball Registration
We are excited to kick off our 2022 - 2023 basketball season and to see our Bearcats on the court again in just a few short weeks.
This year's basketball registration is a 3 step process and all requirements must be completed before the first game. The process is as follows:

Complete this registration form.
- Pay the necessary registration and/or uniform fees (Description of fees and link to pay is below).

Submit a ‘Pre-participation examination’ form found here (State of Illinois Requirement).
Girls Basketball (Grades 5-8)
Registration Fee: $200 (includes league/tournament fees)
Uniform: $50 (includes jersey and shorts)
Boys Basketball (Grades 5-8)
Registration Fee: $200 (includes league/tournament fees)
Uniform: $50 (includes jersey and shorts)
4th Grade Girls Basketball Registration
Registration Fee & Uniform: $200 (includes league/tournament fees, jersey, and shorts)
4th Grade Boys Basketball Registration
Registration Fee & Uniform: $200 (includes league/tournament fees, jersey, and shorts)
If your student's uniform from last year still fits, you do NOT need to order a new uniform.
