Information from School Messenger dated January 8, 2021

Below are a few important reminders as students and staff have returned to campus.

  • If your student is required to quarantine due to a Covid-19 related matter, or if you are unsure, please contact the clinic 547-7313
  • Please see the attached document for our health screening entry form.
  • Students experiencing any of the symptoms on the sheet attached should remain home.
  • Temperatures will continue to be taken in 1st period each day.
  • If your student is currently awaiting a COVID-19 test result they should remain home until receiving their results.
  • If somebody in the same household as your student has tested positive for COVID-19 they will need to quarantine for 10 days from the last day of contact. Please contact the school nurse.
  • The quarantine period may be shortened to as few as 7 days if a negative PCR test result is obtained on day 6 or later from exposure to the positive case. The Dept. of Health will assist in clearing your child to return to school once they have received the negative test result.

Please do not hesitate to contact the school with any questions.

We appreciate your continued assistance in supporting our health and safety protocols here on campus. As a reminder, below is the student health screener questions. If you can say yes to any of the symptoms below for your child, please keep your child at home until you can say no to all symptoms. If you can say yes to the questions, follow the instructions in red.
Student Health Screening Entry Form

Please assess your child daily for the following symptoms and answer the contact questions.

• Fever of 100.4 or higher
• Uncontrolled cough
• Shortness of breath or difficulty breathing
• Sore throat
• Loss of sense of smell or taste
• Muscle aches
• Vomiting or diarrhea

• Is your child currently awaiting COVID-19 test results? If yes, please keep your child home and notify the school nurse when test results are received. Further instructions will be discussed at that time.

• Does your child live in the same household with someone positive for COVID-19? If yes, your child must quarantine for 10 days from the last date of contact with the positive individual.   • June 5 - CLASS of 2021 Graduation - 9:00am at TIAA Bank Field in downtown Jacksonville. We anticipate that families of graduating seniors will be allotted up to 10 tickets. We will continue to work with the venue in creating and communicating any COVID safety protocols. We appreciate your patience and understanding as we continue to work through this process. We are looking forward to celebrating our 2021 Creekside graduates.

Senior Final Exams will be administered May 18th thru May 21st. The last day for seniors who have met all graduation requirements will be May 21st. However, any seniors who have not met graduation requirements will be expected to continue to attend until all are satisfied.

Senior Debt Day is May 28.  

Senior dues must be paid in order participate in Debt Day. Please note specific times for pick-up:

Brick and Mortar students – 9am -12pm
Distance Learner students – 1pm – 3pm

Pick-up for BOTH groups is in front of the auditorium.

Seniors will receive a debt day packet with 10 graduation tickets and any other items needed for graduation.

The NHS will be awarding two $500 scholarships to eligible members of the class of 2021. Graduating members of NHS should complete the Scholarship application and have all required materials submitted to Ms. Cocchiola in portable 14 no later than Friday, April 16. Recipients will be selected by the NHS Faculty Council and will be announced at the Senior Recognition on April 30.

Attention Seniors!

If you are planning to use Bright Futures to pay for summer classes, please read the information below. 

High school graduates seeking immediate summer funding for postsecondary education should try to gain eligibility during their first evaluation by turning in service hours and auditing all requirements. This will ensure that the student is determined eligible before the start of the semester facilitating a smooth transition of summer financial aid. Students awarded a scholarship later in the summer may be required to pay out of pocket for summer classes and be refunded later in the term. 

There will be no extension of tests scores (test taken through June 30, 2021) or service hours. Service hours must be completed and turned in by the last day of school for seniors which is May 21th. 

It is time to start sending your transcripts out to colleges. Before yo do that request a copy of your transcript to review. Things to make sure are accurate on your transcript:

  • correct spelling of name
  • correct address
  • social security number
  • courses taken - including all virtual courses and SJRSC courses
  • correct number of service hours
  • SAT/ACT scores are accurate

To request transcripts, please visit Below are directions on how to create your Parchment account to request and send transcripts. Before requesting a transcript, make sure to update it. This way colleges will see the most up to date transcript available.

Coach Brent Johnson announced his resignation as Creekside’s Head Girls Soccer Coach following the 2020-21 season.
Creekside Athletics extends our sincere gratitude to Coach Johnson for leading the Creekside Knights Girls’ Soccer Program for the past nine years. He truly cared about the success of the players, both as students and athletes. He focused on work ethic and knew how to get the best out of each individual, as evidenced by the program’s tremendous success. Under Coach Johnson’s leadership, the Knights enjoyed multiple district and region championships and two trips to the State Final Four.
Thank you again, Coach Johnson!

Returning Library Books and Textbooks
Students if you have library books and textbooks that you are no longer using please bring them to library to be checked in. You must turn in all checked out items before the end of the school year.

Students can check to see what materials are assigned to them by visiting:
Click Log In and use your S number and Password
Click on “MY INFO” and this will show all library books, textbooks, and fines that are owed to the library.

If you have lost or damaged a library book or textbook, please see someone in the library to find out about your options.

Our ASL 3 & 4 Honors and Access Point Students recently performed and recorded their annual Deaf Awareness Program. The theme this year centered around inspirational / ”making it through” style songs. Great job everyone. Thank you for the wonderful performance.

Students who place in the Top 4 in the State are now eligible to compete at the FBLA National Leadership Conference and Competition this summer.

All preparations were done by these students outside of school, and as you look at the topics, we do not have classes to support many of these events. Our 4th place in Economics was actually by a 9th grade student who has a passion for economics.

Don’t miss your chance to see the Creekside Theatre students shine in Cyrano de Burger Shack. The show features music from the 80’s, 90’s, and 2000’s with songs originally performed by John Mayer, Bruno Mars, Kelly Clarkson, Pink, and more.

The Hispanic Honor Society would like to congratulate sophomore Lily Clarke for winning second place in the Florida Chapter of the American Association of Teachers of Spanish and Portuguese poster contest. Lily represented this year´s Celebrando la diversidad in her digital submission. We are so proud of her hard work and dedication! ¡Excelente trabajo Lily!
Creekside High School is excited to announce that Joe Soto will be the Knights' new Head Girls Soccer Coach.
Coach Soto has been coaching for over 20 years and has coached from recreational to collegiate at all levels. He spent much of his time coaching in northern New Jersey, where he started working with his local recreation program. He went on to coach at the club level, and for over 13 years, has coached girls and boys’ teams.
Coach Soto brings extensive experience coaching at the interscholastic level. He spent seven years coaching girls' high school soccer programs and three years coaching women's college soccer at his alma mater, St. Peters College (NCAA D1). Since relocating to Florida, six years ago, Coach Soto has been a Director of Coaching for Florida Elite. Most recently, he was an assistant coach for Bartram Trail High School's girls' team. During that time, the team won three conference championships, five district championships, four regional championships, and two back-to-back Class 7A State Championships.

Coach Soto currently resides in Saint Johns, Florida, with his wife, Sianna, and their two children Caelen, 11, and Kaia, 9. He is extremely excited to work with the team and looks forward to sharing his passion for the game as part of the Creekside Girls Soccer Program.

Creekside Football is looking for students looking to earn service hours. One or two days a week.

We need assistance with filming practice and games and would love student help building graphics, videos and other items for our social media.

We can take care of all the service hours you need in one season plus you get CHS Football Gear!

Our @creeksidevolleyball program is GROWING‼️

Beach Volleyball is a Spring Sport at Creekside and you could help make our new court a reality! These courts can be utilized for all athletes for strength and agility training. Athletes in the county will benefit from an additional venue to play sand volleyball.

The Creekside Knights Athletic Booster Club (CKABC) is a non-profit organization dedicated to supporting the Knights. We are parents, coaches, students, faculty, administration, alumni, and community members dedicated to promoting excellence in interscholastic athletics. We seek to enrich the high school experience and foster success in the classroom for all students by building school spirit and “Knights Pride” through all sports.

Students - Want to meet with a Dual Enrollment Specialists from SJRSC? 

A DE specialist from SJRSC will be at Creekside every Tuesday during the month of April. If you are a current dual enrollment student, a new student to dual enrollment, or just want to ask some questions about dual enrollment, please use the link below to schedule an appointment with the specialist. These scheduled appointments are for students only. Please schedule the appointment during a time you have an elective period. 

Students new to Dual Enrollment:
Are you wanting to take Summer DE at the college or Fall DE at Creekside or the college? If so, the online application is now open for all students to apply. Below is the link:

If a student wants to take Summer of Fall DE at the college sites, students will need to turn in the registration form, consent form from online application and test scores to Ms. Fornera in guidance by the due dates (which will be determined soon). Summer Dual Enrollment information is out and posted on the guidance Schoology page. The deadline to register for Summer DE is 4/15 at 3:00pm. 

If you need an official test score and are unable to take the SAT or ACT prior to the deadline dates, please see the PERT information attached to register for the PERT exam at the college.

Rising juniors and rising seniors can register for the following dual enrollment classes at their CHS registration appointment, if they qualify.

Rising Juniors - ENC 1101 and ENC 1102
Rising Seniors - ENC 1101, ENC 1102, MAC1105, STA2023, POS1112 and POS 1041

If you are a rising junior or senior and new to dual enrollment (have not taken a DE course previously) and want to register for Fall 2021 DE classes at Creekside, you must meet the following criteria:
·         overall unweighted GPA of a 3.0 or higher at the time of your CHS registration appointment
·         Completed online application AND printed/signed consent form (link is below for online application) - the FALL 2021 application does not come out until on or after 2/15/2021. Please do not complete the online application if you cannot choose Fall 2021 as your start term. Keep checking for when the fall application is out if you are not able to choose Fall 2021 as your start term then you will need to wait to complete the application until you can select Fall 2021 term
·         Official SAT, ACT or PERT (attached are instructions on how to register for the PERT exam at SJRSC) scores - students can check the DE page on the CHS guidance website for qualifying test scores

Creekside registrations will start at the end of February and go through the end of March for their 2021-2022 schedule. Ideally, counselors would like to have students qualify and register for the DE classes they are eligible for at the students assigned CHS registration appointment. This will ensure the student gets placed into the DE class for the upcoming school year. However, students will have until June 10th, 2021 to qualify for DE classes at Creekside. 

If you are a rising senior and currently taking Dual Enrollment classes and you would like to take DE math your senior year - all you need to do is submit qualifying test scores - SAT, ACT or PERT - at your CHS registration appointment.

Registration is open for our on-campus and virtual spring events and tours! Please tell interested students to register online.

We wanted to remind you that the Bright Futures Scholarship application (FFAA) and Free Application for Federal Student Aid (FAFSA) opened on October 1st.

If you are considering applying to college, you should complete both of these applications.

Please see the district financial aid website ( for links to these applications and other resources. 

This school year Creekside students will have the opportunity to reach out to colleges and universities virtually. This means Creekside will not host face-to-face visits on campus. Instead, the link below provides student with admission contact information and virtual tour links to view after the school day.

Virtual information will be updated frequently. However, if you are interested in a college/university, please checkout their websites. More than ever, it is easy to find virtual presentations, tours, and admissions contact information.


(That means allow enough time for car lines and the walk to class after arriving on campus.)
This year, the Absentee Form will be submitted online. Please use the Online Absentee Form link or QR code provided to submit a note when your student in absent. Online absence notes are required to be submitted within 2 days of the student returning back to Creekside in order to be excused.
