Information from School Messenger dated January 8, 2021

Below are a few important reminders as students and staff have returned to campus.

  • If your student is required to quarantine due to a Covid-19 related matter, or if you are unsure, please contact the clinic 547-7313
  • Please see the attached document for our health screening entry form.
  • Students experiencing any of the symptoms on the sheet attached should remain home.
  • Temperatures will continue to be taken in 1st period each day.
  • If your student is currently awaiting a COVID-19 test result they should remain home until receiving their results.
  • If somebody in the same household as your student has tested positive for COVID-19 they will need to quarantine for 10 days from the last day of contact. Please contact the school nurse.
  • The quarantine period may be shortened to as few as 7 days if a negative PCR test result is obtained on day 6 or later from exposure to the positive case. The Dept. of Health will assist in clearing your child to return to school once they have received the negative test result.

Please do not hesitate to contact the school with any questions.

We appreciate your continued assistance in supporting our health and safety protocols here on campus. As a reminder, below is the student health screener questions. If you can say yes to any of the symptoms below for your child, please keep your child at home until you can say no to all symptoms. If you can say yes to the questions, follow the instructions in red.
Student Health Screening Entry Form

Please assess your child daily for the following symptoms and answer the contact questions.

• Fever of 100.4 or higher
• Uncontrolled cough
• Shortness of breath or difficulty breathing
• Sore throat
• Loss of sense of smell or taste
• Muscle aches
• Vomiting or diarrhea

• Is your child currently awaiting COVID-19 test results? If yes, please keep your child home and notify the school nurse when test results are received. Further instructions will be discussed at that time.

• Does your child live in the same household with someone positive for COVID-19? If yes, your child must quarantine for 10 days from the last date of contact with the positive individual. National School Counseling Week 2021 was Feb. 1-5, 2021

This week was created to recognize and celebrate the unique contribution of school counselors.

The tremendous impact school counselors can have in helping students achieve school success and plan for a career is greatly appreciated ALL YEAR LONG!!

We are having a senior class lunch on Friday, February 12th, catered by Chick Fil A at the football stadium. Seniors should report to the stadium during their lunch ONLY.

This lunch will be a great time for our seniors to be together and celebrate the 2021 senior class. Games will be provided for the students to play during their lunchtime. Please note, students will be required to wear masks during this event.
At this time the district has postponed large group photos. This means the senior panoramic scheduled for February 16th has been put on hold. If this changes, we will notify students asap.

Senior cap and gown photos: if students missed it, you may call Leonard’s Studio (904) 239 – 3300 or book an appointment online for 2/20 or 3/20.

Seniors- We have some senior shirts available for sale in room 130 (across from the clinic) for $20. You can bring cash or a check made out to CHS.

Attention Seniors!

If you are planning to use Bright Futures to pay for summer classes, please read the information below. 

High school graduates seeking immediate summer funding for postsecondary education should try to gain eligibility during their first evaluation by turning in service hours and auditing all requirements. This will ensure that the student is determined eligible before the start of the semester facilitating a smooth transition of summer financial aid. Students awarded a scholarship later in the summer may be required to pay out of pocket for summer classes and be refunded later in the term. 

There will be no extension of tests scores (test taken through June 30, 2021) or service hours. Service hours must be completed and turned in by the last day of school for seniors which is May 27th. 

It is time to start sending your transcripts out to colleges. Before yo do that request a copy of your transcript to review. Things to make sure are accurate on your transcript:

  • correct spelling of name
  • correct address
  • social security number
  • courses taken - including all virtual courses and SJRSC courses
  • correct number of service hours
  • SAT/ACT scores are accurate

To request transcripts, please visit Below are directions on how to create your Parchment account to request and send transcripts. Before requesting a transcript, make sure to update it. This way colleges will see the most up to date transcript available. 

If you have questions about your transcript please email your school counselor for assistance. 
Attention all rising Junior and Seniors – the National Honor Society invites all eligible Creekside students to apply for membership.

Candidates must have a weighted GPA of 3.75 or higher, have completed 30 volunteer hours, and must be in good standing with the dean’s office, with no suspensions or infractions. Applications for membership are available in Ms. Cocchiola’s room, P14. NHS will be accepting applications until March 4 for our Spring Induction Ceremony.

Creekside High School Athletics is pleased to announce Estela de Paula as Head Coach of the Knights Girls’ Tennis Team!

Coach de Paula is highly qualified as a tennis instructor and athlete. She grew up playing tennis in Sao Paulo City, played tournaments in South America and Spain, and was top junior ranked in Brazil. She moved to the United States to attend Auburn University with a full tennis scholarship. She earned a bachelor's degree in business administration and reached a NAIA ranking of #2 in the nation.
Coach de Paula is PTR (Professional Tennis Registry) certified and has been teaching tennis to children and adults of all levels for over 20 years. Besides coaching the Knights, Coach de Paula instructs students in Ponte Vedra and St. Johns. She still plays competitively and made it to the USTA 5.0 level National Championship 3 times, with the last competition played in 2018 in Orlando.

She is well known and respected in our community. Her children, Antonio and Julia, are Creekside students.

The Creekside Knights Athletic Booster Club (CKABC) is a non-profit organization dedicated to supporting the Knights. We are parents, coaches, students, faculty, administration, alumni, and community members dedicated to promoting excellence in interscholastic athletics. We seek to enrich the high school experience and foster success in the classroom for all students by building school spirit and “Knights Pride” through all sports.

We have openings and are actively seeking nominees for the 2020-2021 CKABC board. If you are committed to the mission of supporting our athletes, coaches, fans, and staff, please consider serving on the board.

Fall 2021 dual enrollment information at the college site will come out at a later time
Rising juniors and rising seniors can register for the following dual enrollment classes at their CHS registration appointment, if they qualify.

Rising Juniors - ENC 1101 and ENC 1102
Rising Seniors - ENC 1101, ENC 1102, MAC1105, STA2023, POS1112 and POS 1041

If you are a rising junior or senior and new to dual enrollment (have not taken a DE course previously) and want to register for Fall 2021 DE classes at Creekside, you must meet the following criteria:
·         overall unweighted GPA of a 3.0 or higher at the time of your CHS registration appointment
·         Completed online application AND printed/signed consent form (link is below for online application) - the FALL 2021 application does not come out until on or after 2/15/2021. Please do not complete the online application if you cannot choose Fall 2021 as your start term. Keep checking for when the fall application is out if you are not able to choose Fall 2021 as your start term then you will need to wait to complete the application until you can select Fall 2021 term
·         Official SAT, ACT or PERT (attached are instructions on how to register for the PERT exam at SJRSC) scores - students can check the DE page on the CHS guidance website for qualifying test scores

Creekside registrations will start at the end of February and go through the end of March for their 2021-2022 schedule. Ideally, counselors would like to have students qualify and register for the DE classes they are eligible for at the students assigned CHS registration appointment. This will ensure the student gets placed into the DE class for the upcoming school year. However, students will have until June 9th, 2021 to qualify for DE classes at Creekside. If the Fall 2021 application is not out by the time the student h as their CHS registration appointment, they will have to wait until the Fall 2021 application is released and then we can add DE classes – see next line for how the registration will go.

If a student qualifies by the time they register at CHS the will need to bring their consent form from the online application and official test scores to their appointment. If they do not qualify by their CHS registration appointment, counselors will register the student in the HS equivalent course until all required forms are turned in to switch into the DE classes. 

If you are a rising senior and currently taking Dual Enrollment classes and you would like to take DE math your senior year - all you need to do is submit qualifying test scores - SAT, ACT or PERT - at your CHS registration appointment.

We wanted to remind you that the Bright Futures Scholarship application (FFAA) and Free Application for Federal Student Aid (FAFSA) opened on October 1st.

If you are considering applying to college, you should complete both of these applications.

This school year Creekside students will have the opportunity to reach out to colleges and universities virtually. This means Creekside will not host face-to-face visits on campus. Instead, the link below provides student with admission contact information and virtual tour links to view after the school day.
