Information from School Messenger dated January 8, 2021

Below are a few important reminders as students and staff have returned to campus.

  • If your student is required to quarantine due to a Covid-19 related matter, or if you are unsure, please contact the clinic 547-7313
  • Please see the attached document for our health screening entry form.
  • Students experiencing any of the symptoms on the sheet attached should remain home.
  • Temperatures will continue to be taken in 1st period each day.
  • If your student is currently awaiting a COVID-19 test result they should remain home until receiving their results.
  • If somebody in the same household as your student has tested positive for COVID-19 they will need to quarantine for 10 days from the last day of contact. Please contact the school nurse.
  • The quarantine period may be shortened to as few as 7 days if a negative PCR test result is obtained on day 6 or later from exposure to the positive case. The Dept. of Health will assist in clearing your child to return to school once they have received the negative test result.

Please do not hesitate to contact the school with any questions.

We appreciate your continued assistance in supporting our health and safety protocols here on campus. As a reminder, below is the student health screener questions. If you can say yes to any of the symptoms below for your child, please keep your child at home until you can say no to all symptoms. If you can say yes to the questions, follow the instructions in red.
Student Health Screening Entry Form

Please assess your child daily for the following symptoms and answer the contact questions.

• Fever of 100.4 or higher
• Uncontrolled cough
• Shortness of breath or difficulty breathing
• Sore throat
• Loss of sense of smell or taste
• Muscle aches
• Vomiting or diarrhea

• Is your child currently awaiting COVID-19 test results? If yes, please keep your child home and notify the school nurse when test results are received. Further instructions will be discussed at that time.

• Does your child live in the same household with someone positive for COVID-19? If yes, your child must quarantine for 10 days from the last date of contact with the positive individual. Applications for the Miss Creekside 2022 Pageant are due on Monday, March 15th. Miss Creekside 2022 will be crowned on Saturday, May 22nd.

Attention Seniors!

If you are planning to use Bright Futures to pay for summer classes, please read the information below. 

High school graduates seeking immediate summer funding for postsecondary education should try to gain eligibility during their first evaluation by turning in service hours and auditing all requirements. This will ensure that the student is determined eligible before the start of the semester facilitating a smooth transition of summer financial aid. Students awarded a scholarship later in the summer may be required to pay out of pocket for summer classes and be refunded later in the term. 

There will be no extension of tests scores (test taken through June 30, 2021) or service hours. Service hours must be completed and turned in by the last day of school for seniors which is May 21th. 

It is time to start sending your transcripts out to colleges. Before yo do that request a copy of your transcript to review. Things to make sure are accurate on your transcript:

  • correct spelling of name
  • correct address
  • social security number
  • courses taken - including all virtual courses and SJRSC courses
  • correct number of service hours
  • SAT/ACT scores are accurate

To request transcripts, please visit Below are directions on how to create your Parchment account to request and send transcripts. Before requesting a transcript, make sure to update it. This way colleges will see the most up to date transcript available. 

If you have questions about your transcript please email your school counselor for assistance. 
The CHS Studio Art Department will have artwork on display at the St Johns High School County Show in St Augustine, FL at the St. Augustine Art Association. 24 pieces of art from Creekside will be a part of this High School show March 5th through March 28th. The gallery is located at 22 Marine Street, St Augustine, FL
The show is online this year and will open Friday night, March 5th which coincides with the St Augustine First Friday Art Walk.
Congratulations to the Creekside Science Olympiad team for placing 9th at Regionals and moving on to the State finals. This is an amazing accomplishment for a first year team.

Special recognition goes to the following students for medaling in their events:

1st place in Machines: Nicklas Rice (12th)
1st place in Chemistry Lab: Ohm Parikh (12th)
2nd place in Ornithology: Pranav Rajmohan (10th) and Sophia Yu (9th)
3rd place in Experimental Design: Ohm Parikh (12th) and Krisha Shah (10th)

Easter Egg Hunt and Fun

Homeless Coalition
Thought I would let you know a little about the Homeless Coalition. Right now they have 15 homes and 36 children These families must have children in our school district and have been living on the street, car, hotel or other non suitable means. The staff there helps them get back on their feet by helping with employment, which is mandatory to live there, they help teach them about finances, day care on site, tutoring, making sure the children stay at their home school during this 18 months on campus. The ASSIST program at our district will provide transportation for these children to stay at their home school. Without outside support things like Easter Egg Hunts would not happen for these children.
We are collecting plastic eggs stuffed with goodies. Prizes for kids big stuffed animals, boys and girls Easter baskets you can buy or put on together. For the household drawings for grocery gift cards, baskets with household items. Donations may be given to Ms. McNickle.

CHS just won the district battle of the books competition! 

We are so proud of our students! They worked so hard and we’re honored to bring the trophy home to Creekside!

NHS is partnering with the Rotary Club of Ponte Vedra and the St. John’s County chapter of the Learn to Read foundation for a Rubber Duckie Regatta to raise funds for Learn to Read.

Duckies will be sold during lunches beginning March 22, or parents can contact Ms. Cocchiola directly. Duckies are $5 and all proceeds will benefit the Learn to Read foundation. The Regatta will be held on May 16 and winning Duckies are eligible for prizes!

We will be pushing out more information on this fundraiser to include a fun incentive for all NHS members. I’ve attached a copy of the Adoption Card that will be completed for each Duckie purchase so that people can track their Duckie in the Regatta. 😊
Creekside Athletic Physicals @ The Center for Health & Sports Medicine

  • Address: 201 Village Oaks Drive (Corner of SR13 & Racetrack - Behind PDQ)
  • $20 with all proceeds going back to the Creekside Athletic Medicine program.
  • Please bring FHSAA EL2 physical form with 1st page filled out and signed by parent/guardian. We cannot complete the physical if this not done.
  • We will observe social distancing both in and outside of the office. Please arrive at your scheduled time to limit waiting time in the Florida sun.
  • Provided below and via the QR code is the Sign Up Genius link for our Athletic Physicals event at Dr Osborn’s office (The Center for Health & Sports Medicine) on April 24th 1:00 PM – 4:00 PM.

Our @creeksidevolleyball program is GROWING‼️

Beach Volleyball is a Spring Sport at Creekside and you could help make our new court a reality! These courts can be utilized for all athletes for strength and agility training. Athletes in the county will benefit from an additional venue to play sand volleyball.

The Creekside Knights Athletic Booster Club (CKABC) is a non-profit organization dedicated to supporting the Knights. We are parents, coaches, students, faculty, administration, alumni, and community members dedicated to promoting excellence in interscholastic athletics. We seek to enrich the high school experience and foster success in the classroom for all students by building school spirit and “Knights Pride” through all sports.

We have openings and are actively seeking nominees for the 2020-2021 CKABC board. If you are committed to the mission of supporting our athletes, coaches, fans, and staff, please consider serving on the board.

Students new to Dual Enrollment:

Are you wanting to take Summer DE at the college or Fall DE at Creekside or the college? If so, the online application is now open for all students to apply. Below is the link:

At your CHS registration appointment students will need to turn in official test scores and consent form from online application to register for Fall DE classes at Creekside.

If a student wants to take Summer of Fall DE at the college sites, students will need to turn in the registration form, consent form from online application and test scores to Ms. Fornera in guidance by the due dates (which will be determined soon). All information will be available on the Guidance Schoology page as soon as the college releases it. Please check the guidance Schoology often for additional information.

If you need an official test score and are unable to take the SAT or ACT prior to the deadline dates, please see the PERT information attached to register for the PERT exam at the college.


Fall 2021 dual enrollment information at the college site will come out at a later time
Rising juniors and rising seniors can register for the following dual enrollment classes at their CHS registration appointment, if they qualify.

Rising Juniors - ENC 1101 and ENC 1102
Rising Seniors - ENC 1101, ENC 1102, MAC1105, STA2023, POS1112 and POS 1041

If you are a rising junior or senior and new to dual enrollment (have not taken a DE course previously) and want to register for Fall 2021 DE classes at Creekside, you must meet the following criteria:
·         overall unweighted GPA of a 3.0 or higher at the time of your CHS registration appointment
·         Completed online application AND printed/signed consent form (link is below for online application) - the FALL 2021 application does not come out until on or after 2/15/2021. Please do not complete the online application if you cannot choose Fall 2021 as your start term. Keep checking for when the fall application is out if you are not able to choose Fall 2021 as your start term then you will need to wait to complete the application until you can select Fall 2021 term
·         Official SAT, ACT or PERT (attached are instructions on how to register for the PERT exam at SJRSC) scores - students can check the DE page on the CHS guidance website for qualifying test scores

Creekside registrations will start at the end of February and go through the end of March for their 2021-2022 schedule. Ideally, counselors would like to have students qualify and register for the DE classes they are eligible for at the students assigned CHS registration appointment. This will ensure the student gets placed into the DE class for the upcoming school year. However, students will have until June 9th, 2021 to qualify for DE classes at Creekside. If the Fall 2021 application is not out by the time the student h as their CHS registration appointment, they will have to wait until the Fall 2021 application is released and then we can add DE classes – see next line for how the registration will go.

If a student qualifies by the time they register at CHS the will need to bring their consent form from the online application and official test scores to their appointment. If they do not qualify by their CHS registration appointment, counselors will register the student in the HS equivalent course until all required forms are turned in to switch into the DE classes. 

If you are a rising senior and currently taking Dual Enrollment classes and you would like to take DE math your senior year - all you need to do is submit qualifying test scores - SAT, ACT or PERT - at your CHS registration appointment.

Registration is open for our on-campus and virtual spring events and tours! Please tell interested students to register online.

We wanted to remind you that the Bright Futures Scholarship application (FFAA) and Free Application for Federal Student Aid (FAFSA) opened on October 1st.

If you are considering applying to college, you should complete both of these applications.

This school year Creekside students will have the opportunity to reach out to colleges and universities virtually. This means Creekside will not host face-to-face visits on campus. Instead, the link below provides student with admission contact information and virtual tour links to view after the school day.

Virtual information will be updated frequently. However, if you are interested in a college/university, please checkout their websites. More than ever, it is easy to find virtual presentations, tours, and admissions contact information.


(That means allow enough time for car lines and the walk to class after arriving on campus.)
This year, the Absentee Form will be submitted online. Please use the Online Absentee Form link or QR code provided to submit a note when your student in absent. Online absence notes are required to be submitted within 2 days of the student returning back to Creekside in order to be excused.
