On Friday, August 16th from 8:30am-9:15am the PTSO will be kicking off the Class of 2020's final year at Creekside with a special breakfast just for the Senior Knights!

To request a schedule change, students need to submit a request via the schedule change request form online.
 
Due to the number of requests, it may take up to 5 business days to process your request. Requests will be processed in the order that they are received.

The Student Code of Conduct form must be signed by BOTH the student and the Parent AND returned to Creekside by the first week of school before student IDs are issued.

Students may drop off the competed form during Homeroom the first few days of school
or Room 130 in the main office.

Parents - When visiting the front office, please park in one of the many VISITOR parking spaces we have allocated for your use located near the front office and auditorium. Please note, the RESERVED parking spaces are dedicated to Creekside personnel. Thank you in advance for understanding and adhering to the new parking plan.

If you are able to help out, we would greatly appreciate it.

SCHOOL ACCESS IS IMPORTANT @ CHS
***  Volunteers are not the only adults needing school access . ***

Approved School Access ensures the safety of our students, faculty/staff, and our campus. Effective 2015, all visitors and volunteers must have school access. 

 Do you ever???
  • Attend a meeting with Administration or Guidance
  • Attend a PTSO meeting
  • Volunteer time to a Club or Booster on campus
  •  Proctor in the classroom, help in the bookroom, library, etc.
  •  Drive Team members to a game or tournament
  • Volunteer for Special Events (Ticket Sales, Concessions, etc.)
  • Help at a School Sponsored Event (Performance, Camp, Running of the Knights)

If you answered “YES” to any of the above scenarios, it is important to apply for school access online BEFORE your meeting/activity/volunteering. Please go to the school district’s website: http://www.stjohns.k12.fl.us/volunteer/

Please note: You are unable volunteer until your form is cleared in the system. Once cleared, a notification email will be sent to the email address provided on the application. All approvals are valid for three years.

CLINIC INFORMATION
Parents, the nurse will need a Doctor and parent signed medical form for all student’s with chronic health related conditions. If you have questions regarding this requirement, please email Patricia Hardy, BSN, RN at pat.hardy@stjohns.k12.fl.us
The school clinic is located in the main administration building. If your student is unsure of it's location, please tell them to go to the front desk for assistance . Thank you. 
ATTENDANCE INFORMATION
Attendance Policy and Procedure

  • Absence notes are required to be turned in within 2 days of returning back to Creekside in order to be excused.
  • Notes are also required to leave early or check in late. All students must have a planner to check in late or leave early.
  • Student Drivers checking out early MUST turn their note in to the front office by first period.
  • It counts as a day when a student misses 3 or more periods.
  • 5, 10, and 15 day unexcused attendance letters will be generated weekly and Dean Bundshuh will meet with the students who receive these letters.
  • Attendance will be considered as a requirement for privileges.

Parents should contact their student's School Counselor via email to schedule an appointment or for general inquiries. Students may stop by their School Counselor's office during the school day without an appointment. TESTING / PSAT INFORMATION
October 2019 PSAT/NMSQT®
      Creekside High School will administer the PSAT to all students in grades 9-11 on Wednesday, October 16, 2019.

The purpose of the test is to give all students an opportunity to experience taking a college readiness test and to identify students who might benefit from advanced coursework. 
Students in 9th and 11th grade must pay a $21 registration fee in order to take the test and receive their scores.

Students will receive an Official Student Guide to the PSAT/ NMSQT® through their English classes. This guide contains valuable information as well as a full-length practice test with answer keys. Please visit www.collegeboard.org/psat for more information.

To pay the registration fee, p lease make checks payable to Creekside High School and write your student’s name, grade level, and “PSAT” in the memo line. Students may submit registration fees to the Testing Coordinator, Mrs. Mackoul, before or after school in room 130.

The last day to pay for registration will be Monday, September 16 th .

All 10th grade students will receive a score report at no charge. Your student’s Score Report for the PSAT/NMSQT® will be available in mid-to-late December from College Board. 
PLEASE NOTE: Students not taking the PSAT on October 16th will be supervised elsewhere on campus.
CLUB INFORMATION
Most clubs are off and running after school begins leading to the Club Fair which will be held during lunches one day of Homecoming Week. Until then, here is a link from our website: Creekside High School Student Life

Stadium Clear Bag Policy

In the interest of public safety, Creekside High School is implementing a “Clear Bag” policy for those entering the stadium on game day. The new policy intends to enhance security measures and provide a safer game day experience for fans and players alike. Fans are encouraged to bring bags only if necessary. The following bags are permitted, but subject to search:

  • Clear plastic bag or purse that does not exceed 12” by 6” by 12”.
  • Clutch or small purse (with or without a strap), no larger than 6.5” by 4.5”.
  • Equipment bag for necessary medical items.
  • Diaper bag for age-appropriate children accompanied by an adult.

Prohibited bags include, but are not limited to large purses, coolers, briefcases, backpacks, fanny packs, mesh bags, or any bag larger than the permissible size. Guests carrying bags that do not meet the criteria will be asked to return them to their vehicles.

No outside food or drink allowed inside the stadium.

Did you know

When your student calls asking you to pick them up because they are sick, it is imperative they understand they must go to the clinic FIRST for check out.

PLEASE do not arrive and ask us to call them out of class. Instructing your student to go to the clinic prevents class room disturbances and expedites checkout.
