PERS mailed 2018 Form 1099-Rs to all retirees Friday, January 25. If you do not receive your 1099-R by Thursday, February 14, please contact PERS at 800-444-7377. Our offices are open Monday through Friday, 8 a.m. to 5 p.m. CST.

Note:   If the address we have for you is not current, the U.S. Postal Service will not forward your 1099-R. They will, instead, return the form to our office. Contact PERS to update your address and request that we resend your 2018 Form 1099-R.
