Please note there has been a change to Pre-K4 & DK morning carpool dropoff & early dropoff for Pre-K4 & DK. There has also been a calendar update (10/13 is now a half day & 12/20 is a full day). Please make sure to review the perpetual calendar.

Below is an explanation of forms that need to be submitted before the start of school or throughout the school year. All forms can be downloaded from here or from the
school website on the " Parents" tab.

Asthma Action Plan or Allergy Action Plan: If your child has Asthma or a severe Allergy (food, insects, etc.) an Asthma or Allergy Action Plan must be completed by your physician and submitted to the clinic along with the prescribed medication and the Medication Request Form.

The Diocese of Dallas Health History– the immunization record provided by your physician is an acceptable substitute. Immunizations, as required by the Texas Department of Health, must be current for your child to attend class.

Immunization Requirements for the current school year.

Documenting History of Illness: Varicella (Chickenpox) this form should be submitted only if your child had Chickenpox disease.

Medication Permit Form – This form is to be submitted if your child will be taking a prescription medication at school.

Athletic Physical – a physical is required for all students in grades 5th – 8th who are going to participate in sports/cheerleading. The physical must be submitted before participation in the sport. The physical expires one year from the date listed on the form.

Most school supplies are included in your fees. Only a few supplemental items are needed. This information can be found on the " Parents" tab at stmonicaschool.org.

Lunch Program
All students may bring their lunch to school. K-8th grade students also can purchase lunch through our lunch provider, Guess Who's Coming to Dinner.
Pre-K and DK must bring their lunch.
Guess Who's Coming to Dinner will be importing all new students. We will share more details once the import is complete.

Safe Environment Training
is required for any volunteer to be on campus. If you have not done Safe Environment training at St. Monica, now is a great time to start the process.

Band (4-8)
Mark your calendars for the Band Parent Meeting! We are excited to offer Beginner and Concert Band this year.
|

Choir (3-8)
The St. Monica Children’s Choirs are open to children in 3rd through 8th grade. These ensembles meet weekly to rehearse during the school year and sing at several concerts, contests, and liturgies throughout the year. Make sure to check out our parish children's choir too.
|
|
|
|
Chess Club (K-8)
We are excited to begin registration for this year’s chess program for Fall 2023. The Knight School will be leading St. Monica’s chess team this year. Our SMS students will be learning the royal game and enjoying themselves!

|
Bearcat Bots (3-8)
Our robotics club aims to instill STEM education through hands-on Robotics experience. The study of Robotics will incorporate three pillars of STEM - Think (think of ideas on how to design a robot efficiently), Do (build a robot and operate), and Test (test solutions and compete).

Drama Kids (K-5)
We are excited to have Drama Kids International return for the 2023-2024 school year. Drama Kids International will be offering programs for K-5th grades. Students will develop important confidence-building skills that will help them personally and academically.

Junior Author Camp
Bring your imagination to publication through the exciting process of writing and illustrating a professionally published, hardcover book!

Best Book Club Ever
Students will have a great time reading books they like while boosting lifelong skills. Build interest and confidence in reading while exploring genres, exchanging ideas, and enjoying exciting activities!

DJ Geoff will have the party rocking with music and games, and the
SMS Cheerleaders will be supplying the
Bearcat spirit!
We are excited to see everyone! The Knights of Columbus will be grilling burgers and hot dogs outside this year with all the proceeds going back to SMS! Food trucks will also be on hand and will donate a portion of their proceeds to SMS PTO!
As always, you are welcome to brown-bag your own dinner.

What is Autumnfest?
Autumnfest is St. Monica's annual carnival and PTO's only fundraiser. The operating budget for the PTO vastly depends on Autumnfest. Programs such as Catholic Schools Week, Teacher Appreciation, Field Day, Scholarships and Awards, Playground Equipment, School Library, Science Labs, and so much more are made possible because of our success. However, without generous donations and sponsorships from our families and the local community, we lose the ability to offer these programs. Any contributions provided will be greatly appreciated and used for the benefit of the kids!
