Camper Forms are now “live” and we invite you to view the Getting Ready for Camp information on our website! Please review all this information in its entirety. Completing our forms before the April 1 deadline is a critical step to getting ready for camp.

Health Forms

ALL health forms are due on April 1. This includes the Physician Exam Form, Immunization Record, and Health History Form. The Physician Exam Form and Immunization Record can be completed by your doctor based on last year’s physical exam or any visit to the doctor in the past year. 


If your child has not had a physical or seen their doctor since February 2023, please schedule that appointment now so that the form will be in on time.


The Health History Form is an online form that is to be completed by you. You can update it later, as needed, but please let Trudy know if you make any updates after April 1. Our Head Nurse, Maya Levine, begins reviewing all medical forms on April 1, so it is critical that these be in on time.


Pack My Rx registration went live on February 1. You should have received an email with information about registering with them. On our website, you will find all of this information and information on sending medications exempt from Pack My Rx. In addition, there is important information in the Health and Wellness section of Getting Ready for Camp. Just click on Health and Wellness at the bottom. Please contact Trudy if you have any questions about anything mentioned above.
