We are happy to announce we have received small amounts of the Moderna vaccine for the COVID-19 virus and were able to vaccinate the majority of our staff. The PA Department of Health (DOH) has provided vaccines to us which we very grateful for; however, we do not have enough to vaccinate the community per the DOH guidelines. We did create a form for people to sign up if they met the conditions to get the vaccine and within a few days we had nearly 10,000 people sign up. At this time, we took the form off our website because we do not have the supply at this time. Please contact your local hospital’s website and/or your local health department to inquire about signing up to receive the vaccine.
I also wanted to remind everyone of our new location in Barto, PA as we are accepting new medical patients. We are planning to begin dental hygiene services in February so please look out for our announcement when we will begin dental services at the Barto location located at 1315 Route 100 North, Barto. Please check out our website for the latest information and other great announcements!

Working with the Chester County Food Bank, the Care Management Team has been combating food insecurity in our community by facilitating the enrollment, and bi-weekly coordination of fresh foods box deliveries to families homes or to be picked up at the Mall.
The Care Management Team also participated in Zoom Cooking Classes, managed the FVRX program and promoted the use of the Fresh Fruit and Vegetable Truck through the distribution of Veggie Bucks. We look forward to implementing some new initiatives directed at making heart healthy choices this spring.

The Case Management Team is helping our patients get the care they need! Check out this update from Linda Gambone, Supervisor of Case Management here at CHDC:
Community Health and Dental Case Management Team networked with Pottstown Hospital's Community Wellness Program manager in assisting our uninsured patients. Case Management provided the names of 10 uninsured female patients between the ages of 40 and 64 in need of a mammogram.
Tower Health arranged free mammograms to our patients. Out of the 10 females, 2 of them needed further diagnostic testing. Tower Health did not provide additional services beyond the mammogram. Case Management reached out to the Breast and Cervical Cancer Prevention and Treatment (BCCPT) program.
Genuardi Family OB /GYN practice and Einstein Hospital is allotted the funding for the program. The funding allowed our patients to have further testing such as, needle biopsy and post mammograms. Both women are receiving treatment through Einstein Hospital, Genuardi Family OB / GYN, and PA BCCED at no cost to patient.

Are you a female 21 or older? Have you had your pap smear or annual well woman exam? Cervical cancer is the 3rd most common gynecologic cancer. Cervical cancer is caused by the human papillomavirus or better known as HPV. There are many different strains of HPV (40) but about 15 of them are known to cause cancer; subtypes HPV 16 and 18 are found in over 70 percent of all cervical cancers and we screen for these ages 30 and older. We are fortunate in the United States to have access to pap smears which has reduced the incidence and mortality of cervical cancer by 75% over the past 50 years.

CHDC Expands Dispensary Services with Pottstown Pharmacy Partnership
Community Health & Dental Care, Inc. and Pottstown Pharmacy have entered into a relationship to expand pharmacy services and medication discounts to patients in the Pottstown and surrounding communities.
CHDC patients can conveniently get their prescriptions filled either at the CHDC dispensary at the Coventry Mall, 351 W. Schuylkill Road, Pottstown 19465 or at Pottstown Pharmacy, 1501 East High Street, Pottstown 19464.
Pottstown Pharmacy also offers FREE DELIVERY service within our service area. Eligible patients who need access to medication at an affordable price- especially higher costs meds – can receive medication discounts. Eligible patients must be an established medical patient of CHDC and complete the required application to determine healthcare discount to be applied. Eligible patients applying for CHDC’s healthcare discount program may receive medications for as low as $5.00 a prescription. Referrals welcome.

Octodent equipment have been installed in our dental facilities, and patients are being scheduled to allow for appropriate levels of social distancing in our facility. COVID-19 pre-screening includes temperature checks of all patients upon arrival for their scheduled appointment. All patients and staff are required to wear masks.

Community Health & Dental is proud to welcome pediatric medical provider Dr. Priya Venkatesh, MD, FAAP, to the Pottstown health center location at 351 W. Schuylkill Road, Suite G-15A, Pottstown, PA 19465, in the Coventry Mall.
Now accepting new patients, Dr. Venkatesh is a Board-Certified General Pediatrician with over 10 years of professional experience in Primary Pediatric Care and experienced in handling patients from newborn to 20 years. Dr. Venkatesh is also bilingual, speaking both English and Hindi.

Dr. Venkatesh completed her internship and residency in pediatrics at DeVos Children’s Hospital in Grand Rapids, Michigan.
