Big Changes Are Coming to HCCI

We’ve listened to your feedback and combined it with our own research findings to create several new initiatives designed to make it easier for you to work with HCCI and access the important information and resources you need. We will debut a new, user-friendly, more user-focused website. Not only will it look more modern, but it will function that way, too. It will be easier to navigate and find what you’re looking for, provide more statistics and information that can help your day-to-day operations, and be more focused on you and your needs.

A Priceless Simple Gesture

Dr. Paul Chiang, HCCI’s Senior Medical and Practice Advisor and medical director of Northwestern Medicine HomeCare Physicians, shared a story from a recent house call he made which spotlights the critical importance and humanity of bringing healthcare to the home.


What's a battery worth? Well, it depends. 

 

I was originally asked to see an elderly, medically complex patient at home due to his multiple hospitalizations for exacerbation of congestive heart failure. Part of heart failure management is having patients weigh themselves daily and report any weight gain to the provider.

 

After his most recent hospitalization, the hospital sent him home with a scale but without a battery. Previously, the patient told me a friend would help get him a battery "soon.” A week went by, and still no battery.

 

Today, I brought a battery to the patient and installed it in the scale. I reviewed with the patient the importance of getting daily weights.

 

He was so thankful, almost in tears.

 

How much is a battery worth? A few dollars? Thousands of dollars in preventing yet another readmission to the hospital?

 

Gratitude from the patient?

 

Yes, that is priceless.
