2. If you work full-time, part-time, and per diem;
3. The date(s) of your positive test(s);
4. The actual test result(s);
5. The first day you were out of work and the last date before you returned to work;
6. The dates Bishop used PTO to cover the illness;
7. Your check stub(s) showing that PTO was used;
8. The check stub in the pay period before you were out sick, during our illness, and the stub for the full pay period after you returned back to work; and
9. If an incident report was completed, when? If not, why not.
