Dear Bloom Senior Living Residents, Families & Friends:
Our Bloom Senior Living residents' New Year's resolution is to get vaccinated! We're excited to announce that as we close the chapter on a tough 2020, we open a new chapter on a brighter 2021 where we can make that happen. Bloom has been strongly encouraging all its residents and associates to take the highly effective vaccine as soon as its available in their Bloom community. Bloom is receiving a tremendous benefit by receiving priority vaccination status before the rest of the general population. By achieving widespread vaccine acceptance among its residents and associates, Bloom will not only be better able to keep everyone safe, but return to our normalized Bloom life sooner.
Please see the table below to see your Bloom community's vaccine clinic date, time and deadline for submission of consent forms. The consent forms can be downloaded below. Bloom has partnered with CVS Health to administer the vaccine. We'll be updating the below chart and providing updates as further dates are secured from CVS. We're optimistic this will happen very shortly.

Please click on the images below to download the COVID Vaccine Intake Consent Form (all residents and associates must complete) and COVID-19 Responsible Party Consent Form (applicable where resident is unable to complete form or is a minor). In respect to the latter, verbal consent from the Responsible Party is allowed for residents who do not have physical or cognitive ability to consent (or minors). If consent is collected verbally by the community, the Responsible Party must contact the Executive Director and speak directly to them for consent. Voicemails and messages with other community staff is not acceptable. |
