All Bloom Senior Living Vaccination Clinics Have Been Scheduled As Bloom Readies To Move Beyond The Pandemic! |

Dear Bloom Senior Living Residents, Families & Friends:
The first Bloom Vaccination Clinics in South Carolina and Ohio (four communities) have already been successfully completed with high participation rates. Bloom owners, residents and associates came together to be vaccinated and celebrate the beginning of a new day as we move past this pandemic! The clinics had incredible energy and the associates and residents were smiling and engaged. Certificates of Participation we're passed out reading: "I listened to the Science. I want to protect myself, my family, my residents and my coworkers!"
The clinics' rollouts and press coverage, as well as, prior updates regarding the vaccine can be viewed below or on Bloom's Facebook page.
Bloom strongly encourages its residents and associates to join the Bloom movement and GET VACCINATED in order to help us protect our community and start returning to the pre-pandemic Bloom way of life!
Parenthetically, while the vaccine will be administered by CVS Health at Bloom communities, the Centers for Disease Control inadvertently marked Bloom's Louisiana community as Walgreens (instead of CVS). The most efficient path forward in Louisiana is to schedule Bloom at Bossier with Walgreens, which will require our Louisiana Bloomers to download and complete the Walgreens' consent form below (we've also provided a sample / guide to completing the form). All other communities should complete the CVS consent form below as previously advised.
This will be our final Bloom vaccination clinic update. Our global bi-weekly reports will continue.

South Carolina, Ohio, Florida and Indiana communities should kindly click on the images below to download the COVID Vaccine Intake Consent Form (all residents and associates must complete) and COVID-19 Responsible Party Consent Form (applicable where resident is unable to complete form or is a minor). In respect to the latter, verbal consent from the Responsible Party is allowed for residents who do not have physical or cognitive ability to consent (or minors). If consent is collected verbally by the community, the Responsible Party must contact the Executive Director and speak directly to them for consent. Voicemails and messages with other community staff is not acceptable. |
