Reminder:  CBO Panel
Join us this Monday October 3rd, 2016 for the Bronx Health Access PPS CBO Panel!

The goal of the CBO panel is to:
  • Engage and support CBOs in preparing for value-based payment (VBP) 
  • Provide an open forum for sharing experiences, and concerns Learn from CBOs that are currently exploring VBP
  •  Leadership from CBOs that are currently exploring VBP, dealing with the changes coming from DSRIP and health care transformation efforts in NYS will share their experiences and be available for an open discussion.
We encourage partners that are CBOs and not currently receiving Medicaid payments register.  A light lunch will be served.

PMO Updates
UPDATE: Medical Record Review (MRR)

If you have not yet been contacted, you may be contacted by MedReview Inc., a vendor selected by the New York Department of Health (DOH) to complete a chart abstraction of patients within the Bronx Health Access PPS.  This Medical Record Review (MRR) is an integral part of the DSRIP program, providing information on patient outcomes from July 1, 2015 through June 30, 2016 related to the following measures:
·         Childhood immunization status
·         Lead screening in children
·         Prenatal Care/ Postpartum Care
·         Screening for clinical depression and follow-up
·         Comprehensive Diabetes Care
·         Viral Load Suppression
The vendors will select patient charts based on a variety of criteria and reach out to a subgroup of BHA partners to schedule appointments for abstraction services.  
We strongly encourage all partners to prepare to participate fully in the MRR process.
 DSRIP Learning Symposium

On September 20-22, 2016, NYSDOH hosted a State-wide DSRIP Learning Symposium in Syracuse, NY. In keeping with DSRIP's stated goal of restructuring the health care delivery system, the conference was an inspiring event focused on system transformation, clinical management and population health. The symposium also offered opportunities for peer to peer information sharing, and highlighted best practices across program participants. 

BHA was excited to participate along with our partners as both attendees and presenters. The MAX Series Action Team, which focuses on Managing Care for High-Utilizer patients, shared a poster highlighting their innovative work.
Care Coordination lecture by Dr. Shreya Kangovi

On September 29,  the Health Home at Risk project hosted a free lecture on the IMPaCT Care Coordination Model.   The features speaker was Dr. Shreya Kangovi, Founding Executive Director of the Penn Center for Community Health Workers and   Assistant Professor of Medicine at the Perelman School of Medicine, University of Pennsylvania .  Dr. Kangovi facilitated and interactive discussion and shared best practices from implementing "IMPaCT" (I ndividualized Management towards Patient-Centered Targets) .     The IMPaCT model incorporates  patient perspectives on the socioeconomic determinants of their health and healthcare utilization and the effectives of the Community Health Worker in modifying these determinants .  For more information on the IMPaCT model--

IT Committee

The Bronx Health Access PPS and Bronx RHIO was invited to present at the Second Annual  DSRIP Learning Symposium in Syracuse, NY.  Steve Maggio, Senior Project Manager, BHA, and Keela Shatzkin, Bronx RHIO, presented and participated in a panel discussion around effective collaborations between PPS's and Qualifying Entities(QE) showcasing the work involved in developing the new Care Coordination Clearinghouse.The presentation was very well received by other PPS's and NYS officials.

The IT Committee continues to aggregate patient data for Actively Engaged metrics and continues to further develop and operationalize the Clearinghouse.

The IT Committee and Bronx RHIO analytics build continues to progress with the second wave of metrics being released to the PPS in October.
Workforce highlight

BHA would like to congratulate a cohort of 9 students who have successfully completed the Spanish Language of Care course sponsored by the Workforce Committee. The Language of Care courses aim to enable healthcare staff to communicate with their non-English speaking patients and family. The course also encompasses a cultural awareness module intended to help caregiver understand the cultural beliefs and customs of their patients. The program was offered on-site at the Fulton campus and Grand Concourse campus. 

Reminder: All staff members must complete the DSRIP 101 e-learning course.
Bronx Health Access and 1199SEIU Training & Employment Funds have developed a free online DSRIP (Delivery System Reform Incentive Payment) interactive course.  This 30 minute e-learning course, titled DSRIP 101, is designed to teach front line and managerial staff the goals of DSRIP and the reforms occurring in the New York health care system. The New York State Department of Health requires all PPS partners and their employees to complete this course. Some of the topics covered in the course will be:
  • Why is healthcare changing?
  • What is DSRIP?
  • What is a Performance Provider System?
  • What is the triple aim of healthcare?
  1. Click on the "Sign Up Now" button above
  2. Click "Take this course"
  3. A user login box will appear, click "not registered"
  4. A user registration box will appear, fill out all the fields and click submit (write down your username and password)
  5. It will prompt you to put in your username and password you created
  6. Your profile will be created and your first and last name will appear. You will fill out all empty fields and click submit.
  7. Under course content click BHA PPS DSRIP 101 video and you should be able to see the video
  8. Please remember to check off the "Complete" box once you are done.
