CHHA, Hospice and LHCSA HERDS Surveys Now Open

The State Department of Health (DOH) has announced this morning that its COVID-19 LHCSA, CHHA and Hospice HERDS Survey is now open with reporting due by 2:00 PM today. 

DOH requires companies or organizations with more than one license number to complete the survey for each license number. All LHCSAs are expected to submit the survey except for those LHCSAs solely serving Assisted Living Programs. 

The DOH requires the survey by 2:00 p.m. each Tuesday and stresses that agencies that fail to respond may be referred for enforcement which can include fines of $2,000 per occurrence. Agencies that wish a copy of the day’s report should print a copy before submission. 

DOH asks that inquiries related to COVID-19 and the HERDS Surveys, including difficulties with submission, should be referred to the following program-specific email address: hcstatrpts@health.ny.gov. 

Effective June 17, 2025, LHCSA providers will have three days to complete and submit the survey to comply with the existing reporting requirement. The reporting cycle for all LHCSA providers will begin on Tuesdays at 8:00 a.m. and will close on Thursdays at 2:00 p.m. In addition, a new two-part question has been added to collect the provider’s patient census by county. These new changes will remain in effect until further notice from DOH.
