CHNA 6 Updates:
Bidders Conference:
Please consider joining us at the Bidders Conference to learn more about that CHNA 6 Grant Process and an opportunity to ask questions. The Conference will be held on Friday, January 6, 2017 from 10-11:30 am at Milford Regional Medical Center, 14 Prospect St. in conference rooms  A & B.  Please park in the Cancer Center and cross at Prospect St to the main lobby.  Take a left in the lobby and the conference rooms are on your left. There will be coffee and bagels! 

We would also like to extend a special thank you to Ellen Freedman, as she has so skillfully guided and supported us over these past years and particularly for her consulting work over the past months to help get us to this point. We wish her the best of luck in her future endeavors. Ellen can be reached here for all future correspondence. 
Grant information:
In keeping with the recommendations of its 2015 Community Health Improvement Plan, the Greater Milford Community Health Network Area (CHNA 6) is pleased to solicit proposals from eligible nonprofit and government organizations for the following four Priority Areas:
  1. Chronic Disease Prevention and Health Promotion
  2. Behavioral Health and Substance Abuse
  3. Violence Prevention
  4. Access

Upcoming Dates:

    • Please consider bringing a gift card of denominations of $10 to be donated to Catholic Charities. They are in need of gas, grocery store, Target and Walmart gift cards.
  • February 16, 2017: Grant propsals due by 4 PM

Greater Milford Community Health Network (CHNA 6) is one of 27 Community Health Network Areas (CHNAs) across Massachusetts. We serve the 13 communities of: Bellingham, Blackstone, Douglas, Franklin, Hopedale, Medway, Mendon, Milford, Millville, Northbridge, Sutton, Upton and Uxbridge.  CHNA 6 is a partnership with the Massachusetts Department of Public Health, the Milford Regional Medical Center, local service agencies, schools, businesses, Boards of Health, community health centers and other health organizations, as well as other concerned citizens who are working together to build healthier communities.
