Dear CHNA 6 Member,
Welcome to our monthly CHNA 6 Newsletter! We are hoping that this newsletter will provide our dedicated members with information to better connect you to our growing network.

Upcoming Meeting and Panel:
A reminder for our next CHNA 6 Meeting! The meeting will be Friday, June 10, 2016 at the Ruth Anne Bleakney Senior Center, located at 60 N. Bow St., Milford, MA, from 9-11 AM. We will have a panel
presenting "Diabetes Prevention and Management" to include:
- Dr. Mariela Sandalis, staff physician and Coordinator of Diabetes Prevention Program at Latino Health Insurance Program
- Kimberly Cohen, Senior Director of Health Innovations at Hockomock Area YMCA
- Jo Fleming, Diabetes Educator, Milford Regional Medical Center
- Denise Coyle, Nurse, Salmon VNA and Hospice
We would also like to congratulate all the awardees of a 2016 CHNA 6 Mini-Grant! For complete list of awardees and projects, click here. Also, congratulations to the Town of Milford on the support at Town Meeting for a Fixed Loop Bus Route.

Fall Meeting Dates:
CHNA 6 Meetings will be held next Fall at the following dates and locations, from 9-11 AM. Our upcoming meetings are:
- Friday, September 16, 2016 at Ruth Anne Bleakney Senior Center
- Friday, November 4, 2016 at Milford Regional Medical Center
- December Holiday Party: TBA
FYI: Attendance is required at 50% of meetings for those receiving CHNA 6 grants

Bellingham, Blackstone, Douglas, Franklin, Hopedale, Medway, Mendon, Milford, Millville, Northbridge, Sutton, Upton and Uxbridge. CHNA 6 is a
partnership with the Massachusetts Department of Public Health,
the Milford Regional Medical Center,
local service agencies, schools, businesses, Boards of Health, community health centers and other health organizations, as well as other concerned citizens who are working together to build healthier communities.
