CHNA 6 Updates:

The upcoming CHNA 6 meeting on Friday, November 4th from 9-11am is to review the Greater Milford Community Health Improvement Plan (CHIP) after our first year of implementation. We hope to learn about the work of funded projects and identify opportunities for collaboration and future funding.  All are encouraged to attend!  For questions, please contact Ellen at e.freedman@chna6.org


Please note location:The meeting will be held at Milford Regional Medical Center, 14 Prospect St. in conference rooms  A & B.  Please park in the Cancer Center and cross at Prospect St to the main lobby.  Take a left in the lobby and the conference rooms are on your left.


The steering committee meets every other month to set agenda for general CHNA 6 meetings, approve recommendations of the grants committee, and members serve on one of the subcommittees; finance, outreach/education or grants distribution. The commitment is about 10 hours a month. If you are interested, please submit this brief application by November 7th to coordinator@chnna6.org

  • January 6, 2017: 10-11:30am - Bidders Conference
  • January 13, 2017: 9-11am - Holiday party and meeting
  • February 16, 2017: Grants due (large and mini grants)
  • March 10, 2017: 9-11am -  CHNA 6 meeting
  • March 31, 2017: Grants awarded
