La Clinica del Pueblo completes its three-year grant that supported a New Access Point in Hyattsville

La Clinica del Pueblo completed its three-year grant with the CHRC that supported a new Federally Qualified Health Center site in Hyattsville, Prince George's County. La ClĂnica del Pueblo is a bilingual, bicultural, community health center that provides comprehensive care for the Latino community including primary care, mental health and substance abuse services, HIV/AIDS prevention, and community health outreach. Over the course of the three-year grant, La Clinica has leveraged CHRC funding to receive an additional $857,000 from private and local sources. Family Tree leverages CHRC grant to receive additional $200,000 for Family Connects Maryland

Family Tree, a community based non-profit organization in Baltimore City, received a $200,000 grant from the Maryland Department of Health to support the Family Connects Maryland program. The Family Tree program, originally supported by a $150,000 grant from the CHRC, is a brief home visiting program for new mothers that is designed to improve infant and maternal health and well-being by providing postnatal education and support in the home and links new mothers and their families to needed resources. The program is being piloted in Maryland in partnership with Sinai Hospital, Duke's University's Center for Family & Child Health, and the University of Maryland School of Social Work.

During the two-year CHRC grant period, Family Tree served 490 new mothers, with 76% of these mothers linked to at least one needed community resource. The program performed a total of 537 home visits. The additional funding for the program provided by the state will support an expanded evaluation of the impact of the intervention.

The Commission has awarded 24 grants totaling $4.7 million to increase access to women's health services and help address infant mortality. These grants have supported programs in rural, urban, and suburban communities. For more information about CHRC's women's health and infant mortality grants, click here.
