JULY 2020 NEWS and UPDATES
As we celebrate the 30th anniversary of the signing of the Americans with Disabilities Act (ADA), let's reflect on how far we've come, and also give thought to how much farther we need to go to ensure access for people who are Deaf, DeafBlind and Hard of Hearing. Please consider a donation to CHS to celebrate the ADA and the programs and services that we have to offer.
Audiology Clinic Accepting In-Office Appointments
The CHS audiology clinic is now seeing patients, in our office, Monday-Friday, by appointment only. Please call us at 773.248.9121 or email Leticia to schedule your hearing test, selection, dispense, repair, or to order/pick-up supplies.

We are taking extra measures to keep our patients safe. Upon arriving for your appointment, you will be asked a few questions about your recent health. You may also be asked to sanitize your hands upon arrival. Your temperature will be checked upon arrival (including children). Depending on your temperature, you may be asked to reschedule. If you require additional assistance for your appointment, one additional person may be allowed to accompany you (all others must wait outside of our offices).

Please have your ID and insurance card with you. We look forward to serving you.
