Updated Patient Applications
We've uploaded new versions of the Uniform Application (Version 1.5) and Clinic Client Application (Version 10) to the CICP Provider Information webpage and the HDC Provider Information webpage.
The only change to the applications is the removal of the CICP Only statement on the application tab related to CICP applicants refraining from executing an affidavit of support for the purpose of sponsoring an immigrant.
This change is due to the passage of House Bill 23-1117, which also removes this stipulation for patients applying for public benefits, including Health First Colorado programs. Please begin using this version of the application immediately. Any questions may be sent to [email protected].
