December 2021
As the holiday season quickly approaches, we want to express our sincerest gratitude for your continued support and participation in San Diego's Community Information Exchange. Together we've accomplished so much to be proud of this year.

The small (but very mighty) 211/CIE San Diego team will be taking time off to reflect and spend time with family and friends. During this time (12/24 - 1/3) there may be a delay in our response. Please utilize our website to access our online training and guides for troubleshooting your password and more. Staff will be available for emergencies at 858-300-1224.

Thank you for your partnership and support as we work together to serve every person in every community with respect, and dignity.

We look forward to providing a smooth and seamless support experience for you once we are back in the office.

We wish you and yours all the best this holiday season and are so excited for our work together in 2022!

NEW RELEASE
NEW RESOURCE: LEVERAGING COMMUNITY INFORMATION EXCHANGES FOR EQUITABLE AND INCLUSIVE DATA: CIE COMMUNITY PROFILES
We are excited to share a new resource, CIE Community Profiles, which was produced as part of our project, Leveraging Community Information Exchanges for Equitable and Inclusive DataCommunity members are the most critical element to the successful design and implementation of a Community Information Exchange (CIE)®. As we mentor communities across the county working to improve health through local data sharing, we want to highlight organizations at different stages of the design and implementation of a CIE. The CIE Community Profiles provide local approaches to uplifting community members and incorporating Community Voice as a part of striving towards health equity.
This work was made possible with funding support from the Robert Wood Johnson Foundation.

CIE Partner Gathering

The CIE Partner Network works to improve the overall service delivery system for clients. Our meeting provides a forum for partners to share updates about ongoing CIE Initiatives, share trends in our community, and collaborate on how we as a network are leveraging our partnership, data sharing, and the CIE technology platform to address the social determinants of health and increase health equity.

Join us for a range of content. Each month we may dive into the latest technology projects and elicit feedback on the platform’s care coordination tools. We may spotlight programs and supports for unique populations. The partner gathering is a space to engage with peers across our shared network. Join us on the 4th Thursday of every month (except November and December) at 9:00 am via Zoom.
