Immigrant Heritage Month

Immigrant Heritage Month is a month-long celebration that allows communities to explore their own heritage and celebrate their shared diversity. 
It was proclaimed Immigrant Heritage Month in June of 2021 by President Joe Biden in which he called the people of the United States to learn more about the history of immigrant communities throughout the generations following the Nation’s founding and to observe this month with appropriate programming and activities.

Juneteenth Annual Holiday

Juneteenth (short for “June Nineteenth”) marks the day when federal troops arrived in GalvestonTexas in 1865 to take control of the state and ensure that all enslaved people be freed. The troops’ arrival came a full two and a half years after the signing of the Emancipation Proclamation. Juneteenth honors the end of slavery in the United States and is considered the longest-running African American holiday. On June 17, 2021, it officially became a federal holiday.

The City of Vista Social Work team comprises of one care manager who provides case management services such as coordination of care, physical health, mental health, substance abuse, county benefits, and community referrals as well as one housing navigator who assists clients with finding, getting, and maintaining appropriate housing. Services are primarily offered off-site and in the community.

Cool Zone Assisted Transportation Service

FACT offers Assisted Transportation Services to and from cool zone locations within San Diego County. This service is most appropriate for persons with mobility needs, examples include but are not limited to:

  • Needing assistance to/from their residence or facility
  • Persons utilizing a folding or nontransferable wheelchair
  • Persons who otherwise require additional assistance such as special vehicles to accommodate medical devices or equipment.

Aligning California: Maximizing Opportunities to Advance Local Community-Led Networks

California is leading the nation in community health and systems change innovation. Recent local and state policy proposals prioritize funding to support data-sharing infrastructure, with an emphasis on technology-enabled person-centered care models. The Community Information Exchange (CIE) model can be used to design systems that meet the needs of Medicaid recipients and other vulnerable populations while prioritizing local community governance.

We’re bringing together CIE organizations and other stakeholders from across California to explore how we can leverage the CIE to achieve systems change and prepare for future funding opportunities that support community health.

CIE Partner Gathering

The CIE Partner Network works to improve the overall service delivery system for clients. Our meeting provides a forum for partners to share updates about ongoing CIE Initiatives, share trends in our community, and collaborate on how we as a network are leveraging our partnership, data sharing, and the CIE technology platform to address the social determinants of health and increase health equity.

Join us for a range of content. Each month, we may dive into the latest technology projects, elicit feedback on the platform’s care coordination tools, or spotlight programs and supports for unique populations. The partner gathering is a space to engage with peers across our shared network. Join us on the 4th Thursday of every month (except November and December) at 9:00 am via zoom.
