Referral Management Dashboard Upgrade
  • With the new referral management dashboard, you will be able to make bulk updates to direct referrals.

Case Management Dashboard  
  • The feature is customizable, so you can quickly review your client base faster than ever before. With one click, view a list of the clients your organization that consented into CIE or a list of your recently viewed clients. 

Alerts  
  • Care Team Members will soon receive notifications of housing and health changes, an indication of food insecurity, and missed appointments for CIE participants to provide proactive support.

Ability to add Household Information
  • You will be able to list other individuals that are in the client’s household, including Co-parents, children, and Authorized Representatives

Referral Management E-Learning Module
  • This module covers the lifecycle of a direct referral, how to navigate the Referral Dashboard, and the steps to close out a referral from beginning to end.

CIE Platform Access
To access the assessment and other tools available in CIE, you are required to complete CIE Care Coordination E-learning and complete a short quiz at the end. To access the training, please contact our CIEhelpdesk.

County Homelessness Lived Experience Forums
On behalf of the County, San Diego Housing Federation is holding a series of sub-regional forums for individuals with lived experience of homelessness to provide input into the design of homeless and housing solutions for their sub-region of San Diego County. Participants will be provided with a stipend for their participation. Reports will be kept anonymous and shared with the County to use for planning. Feedback forums will focus on topics of emergency shelter, safe parking/sleeping site(s), interim/permanent housing, and Day Center access.

To participate, individuals must:
  • Sign-up online
  • Have lived experience of homelessness
  • Live in the sub-region to participate, and can only attend one session

Participants will receive a gift card stipend.

FORUM DATES

EAST COUNTY:
Covering the areas of El Cajon, Lemon Grove, La Mesa, Lakeside, Santee Spring Valley,
Poway, and unincorporated east county bordering these cities.
Meeting Location: TBD
  • Wednesday- October 27 at 12:00 pm
  • Wednesday- November 10 at 6:00 pm

SOUTH COUNTY:
Covering the areas of Imperial Beach, Chula Vista, National City, and unincorporated
south county, including Bonita, Lincoln Acres and other communities bordering these areas.

September is Hunger Awareness Month
1 in 3 people in San Diego County faces hunger. Together, we can create a Hunger-Free San Diego. Hunger Awareness Month is an annual initiative organized by the Hunger Advocacy Network, a collaborative of 19 organizations advancing policies to end hunger in San Diego County. This year, they will highlight solutions to hunger faced by many San Diego residents, building public awareness and support to improve nutrition security and hunger relief in San Diego County. The City of San Diego in partnership with The San Diego Foundation is pleased to provide the Small Business and Nonprofit Relief Fund. The City of San Diego, thanks to funding from the American Rescue Plan of the Federal Government and local funding from The San Diego Foundation, is pleased to offer grants for City of San Diego small businesses and nonprofits.

The deadline to apply for the City of San Diego Small Business and Nonprofit relief fund is Friday, Sept. 24, 2021, by 5:00 p.m.

CIE PARTNER GATHERING

The CIE Partner Network works to improve the overall service delivery system for clients. Our meeting provides a forum for partners to share updates about ongoing CIE Initiatives, share trends in our community, and collaborate on how we as a network are leveraging our partnership, data sharing, and the CIE technology platform to address the social determinants of health and increase health equity.

Join us for a range of content. Each month we may dive into the latest technology projects and elicit feedback on the platform’s care coordination tools. We may spotlight programs and supports for unique populations. The partner gathering is a space to engage with peers across our shared network. Join us on the 4th Thursday of every month (except November and December) at 9:00 am via Zoom.
