NEW DIRECT REFERRALS
2-1-1 San Diego
Provides assistance to clients who are 65 and older with scheduling a COVID-19 vaccine appointment.

  • Must be residents in San Diego County
  • Direct Referral can be made in the CIE Partner Community
Interfaith Community Services

Offers temporary rental assistance available to assist households whose employment income has been negatively impacted by COVID-19. Households must demonstrate an inability to meet rent obligations by documenting a loss of income and lack of assets available due to COVID.
  • Serves individuals and families whose income is below 50% AMI, are at risk of experiencing homelessness, income was affected by COVID-19, and reside in the City of Escondido.
MAAC Project
Serves children (0-5) and their families, children with disabilities, and pregnant women that reside in North San Diego County in the communities of Carlsbad, Escondido, Fallbrook, Oceanside, San Marcos, Valley Center, Vista, and San Ysidro.
SBCS CORP (AKA) South Bay Community Services
Group-Based Parenting Classes offer parents the ability to learn about their child’s development, new parenting techniques and skills, and to receive support from other parents.
Vets Community Connection
  • Offers local career networking opportunities for veterans, active-duty military, their spouses, and caregivers. Participants will have the opportunity to meet with local employers and get tips on how to gain employment.


Provides opportunities to volunteer in support of local veteran and military programs.

CIE SAN DIEGO SELECTED TO LEAD THE RESEARCH ON EQUITY-DRIVEN APPROACHES TO CARE
211/CIE San Diego received a grant from the Robert Wood Johnson Foundation (RWJF) to help advance community and public health data transformation and health equity. Through the grant, 211/CIE San Diego will collaborate with other key stakeholders to co-author reports that provide practical examples of the community-driven Community Information Exchange (CIE) model; identify challenges and promote the opportunity to create anti-racist data systems, and establish a strategic national agenda to support communities building CIEs to reimagine public health data and community needs.

COVID-19 UPDATE
211 San Diego Supporting the County's COVID-19 Vaccination Efforts
211 San Diego is currently supporting County in information for COVID-19, including general information, testing, and vaccination.
  • Educate on phases (phases to be expanding soon)
  • Navigating County/Community locations for vaccine appointments
  • Assisting 65+ on setting vaccine appointments without internet or supports to make appointments
  • Role with equity and access to ensure populations can get the vaccine and communicating issues and opportunities with the County
  • Partnering with organizations to serve hard to reach populations
  • 2-1-1 is available 24 hours a day, 7 days a week
  • Serves more than 200 languages

CIE MONTHLY TRAINING OPPORTUNITIES
The CIE Team is offering monthly training on the 2nd Wednesday of every month from 10:00 am to 11:00 am via zoom! The next Training is on April 14, 2021. Learn how to navigate the CIE platform, search for clients, send direct referrals to other partners, how to add program enrollments, care teams, and more.

CIE PARTNER MEETINGS
The CIE Partner Network works to improve the overall service delivery system for clients. Our meeting provides a forum for partners to discuss the CIE platform, provide feedback about the CIE platform, and participate in ongoing discussions and improvements to the social determinants of health, risk rating scales, system functionality, as well as network with other CIE partners to coordinate better care for clients. We meet on the 4th Thursday of every month (except November and December) at 9:00 am via zoom.
