Hey CIE Partners,

Happy autumn equinox! Thank you for attending the CIE partner gathering on September 26, 2024. At the start of the meeting, we paid homage to Karis Grounds, a trailblazer of the CIE movement. Throughout her 11 years with 211 San Diego, Karis dedicated herself to enhancing social and healthcare systems through our partnerships at 211 San Diego and the Community Information Exchange. We will miss her and extend our best wishes for success in her new role at Elevation Health Partners, a consulting firm committed to advancing value-based care and system improvement.

We also celebrated National Hispanic Heritage Month and enjoyed hearing everyone's favorite cultural food dishes!

We introduced one of our new CIE Partners, San Diego Voluntary Organizations Active in Disaster (SDVOAD)! They are dedicated to enhancing the efforts of member organizations through interagency planning, relief, response, and recovery activities, all aimed at providing area residents with timely and impartially delivered services in the event of a disaster.

We also highlighted the Neighborhood House Association's Care Link program, a Medi-Cal managed care benefit that focuses on addressing the holistic needs of enrolled members. The program offers both Enhanced Care Management (ECM) and Housing Navigation.

Joanna Oboza from the 211/CIE San Diego team guided us through the process of accessing Enhanced Care Management screening by logging into the CIE Partner Community and explained how to connect clients in the communities to services that can help using CIE. If you have individuals or children potentially eligible for Enhanced Care Management, you can refer them to 2-1-1's Enhanced Care Management team. The team will help facilitate a referral to an ECM provider in the community or directly to the health plan if needed.

Jesus Aguilera from the 2-1-1 San Diego resource center team provided a brief demonstration on how to update your services in the 211/CIE San Diego resource database to ensure that clients receive accurate information about your programs and services.
