CIE PARTNER MEETING RECAP

Thank you for joining the September 2025 CIE Partner Gathering virtually. We welcomed 52 participants from our community and discussed a variety of relevant topics.


We started the meeting by raising awareness for National Suicide Prevention Month, celebrating Hispanic Heritage Month, and marking the arrival of the Autumn Equinox. This season includes several celebrations, such as Halloween, the enjoyment of sweater weather, and the delight of pumpkin-flavored treats. Our partners shared their favorite ways to celebrate the Autumn Equinox, which included hot chocolate, champurrado, apple picking, trips to Julian, and more.

We welcomed Care Partners as the latest CIE Partner, and Tina Roth, their Enhanced Care Manager, provided insights on how partners can collaborate with their organization. She also provided an overview of their programs and services, which include Enhanced Care Management (ECM) and a new series of Community Support services, including Personal Care Services and Respite Services. These services are now available as closed-loop referral opportunities in CIE.

 

Helping Hands San Diego Foundation also added five closed-loop referral opportunities to support seniors or individuals with disabilities.

Aidee Roman shared updates on CIE's strategic goals for fiscal year 2025–2026, focusing on increasing platform utilization through expanded referral services, service-level agreements, and tracking outcomes. She highlighted several key objectives, including ensuring that closed-loop referral options are effective, promptly acknowledging referrals, and connecting clients to the necessary services.

 

To accomplish this goal, the 211/CIE San Diego team will be conducting multiple activities to collect partner feedback. These activities will include developing a list of the most desired referrals, closely monitoring SLAs, sharing partner utilization reports monthly, sending email reminders to partners with pending direct referrals, and providing updated training tools and materials to partners. These efforts aim to strengthen referral flows, foster accountability and trust within the CIE network, and create a more connected and responsive system of care.

 

To kick things off, partners were asked to complete a poll during this meeting to identify their top closed-loop (direct) referral partners and express their interest in learning more about direct referral processes. The information gathered from the poll, along with feedback from partners, will help shape future initiatives, including a closed-loop referral matching activity planned for the upcoming CIE Partner Gathering in October. San Ysidro Health, a longstanding CIE partner, shared their experiences and processes for managing direct referrals within CIE. They provided real-life examples demonstrating how partners can streamline their referral processes using CIE.


Angie Esparza-Banuelos and Keila Gomez from the Partner Experience team presented a demo on how to effectively close the loop on pending and active direct referrals using CIE.
