Announcing CIE Platform Enhancements:

14 Social Determinant of Health Assessments - Now Available


The Comprehensive Social Continuum Assessments (CSCA) tool consists of 14 assessments that evaluate risk, help with care planning, and prioritization of resources. Each assessment measures:


  • Immediacy of Need: how soon does the client need help?


  • Client’s knowledge and utilization of services: does the client know about resources in the community and have they used them?


  • Present barriers and/or supports: are the client’s limitations or barriers preventing access? Does the client have any existing social supports? 


A completed assessment produces one of the following risk-rating scores:

Closed-Loop Referral Spotlight:


Provides a 12-month, faith-based, residential recovery transitional shelter program for men, women, and single parents. Using a holistic approach, the program addresses mental, physical, and spiritual needs, helping individuals overcome homelessness and achieve long-term self-sufficiency. Participants live on-site for the duration of the program and receive personalized support.


Services include:

  • Legal assistance
  • Parenting support
  • Health care referrals
  • Domestic violence support
  • Job skills and vocational training
  • Individual and group counseling
  • Addiction and recovery education
  • Anger management and conflict resolution


Eligibility (Must meet the following):


  • Men, women, and single parents (with up to 4 children, ages 2–17) who are homeless or at risk of homelessness
  • Willing to commit to the 12-month, faith-based recovery process and actively participate in all three phases of the program
  • Able to live and work in a community environment
  • Pass a drug test and agree to maintain a sober living environment
  • Capable of performing Activities of Daily Living (ADLs) independently
