We have kicked off our 2025-2027 CIN cycle and are thrilled to announce two upcoming CIN webinars designed to help our CIN members and partners transform care, improve outcomes. and advance health equity. 

 

On May 27th CIN will be hosting a webinar designed to help organizations implement CIN’s Toolkit to Advance Racial Health Equity in Primary Care into FQHC settings. In June, CIN will be hosting a webinar that introduces organizations to practical strategies for incorporating equity into the Model for Improvement, leveraging lessons from a DHCS/IHI (California Department of Health Care Services/Institute for Healthcare Improvement) Child Health Equity Improvement Collaborative. UPCOMING CIN WEBINAR: Practical Application of the CIN Racial Health Equity Toolkit to Advance Racial Health Equity in Primary Care


May 27th, 2025: 12-1pm PT 

 

Advancing health equity in primary care is both an outcome and a process that requires continuous learning and improvement. The CIN Racial Health Equity Toolkit, developed in partnership with HealthBegins, is designed to help organizations who provide, pay for, or support primary care translate their verbal commitment of racial health equity into organizational practice. 

 

In this CIN webinar, led by Elevation Health Partners, we’ll discuss the CIN toolkit for advancing racial health equity and hear how Elevation Health Partners is using the PATH TA Marketplace to support Health Centers to initiate health equity improvement efforts using the CIN toolkit. 

 

At the end of the webinar participants will: 

  • Understand how to implement the California Improvement Network’s (CIN) Toolkit to Advance Racial Health Equity in Primary Care in an FQHC setting. 
  • Learn the key drivers for advancing racial equity in primary care including steps to create a foundational understanding of racism and racial health equity. 
  • Hear ways to prioritize drivers that can advance racial health equity within their own organization. 

UPCOMING CIN WEBINAR: A Model for Equity-Centered Quality Improvement

Quality Improvement (QI) is a powerful tool for transforming care delivery and improving outcomes. Too often however, equity is not explicitly addressed within standard QI approaches as many common tools and methods focus on efficiency and outcomes without examining who benefits or who might be left behind. As a result, existing disparities can persist or worsen despite well-intentioned improvement efforts. 

 

To drive meaningful, lasting change it’s essential to embed equity into every step of the QI process. This CIN webinar is designed to introduce organizations to practical strategies for incorporating equity into the Model for Improvement, leveraging lessons from a DHCS/IHI (California Department of Health Care Services/Institute for Healthcare Improvement) Child Health Equity Improvement Collaborative. 

 

At the end of the workshop participants will:  

  • Understand approaches to address equity within the context of quality improvement projects, particularly within pediatric care. 
  • Hear how managed care plans and their partners applied equity-focused QI methods to improve well-child visit rates.  
  • Get inspired to try new quality improvement methods and approaches within your organization.

In the second installment of Center for Care Innovations AI in Action: Practical Application for the Safety Net webinar series, Dr. Hakeem O. Adeniyi, Jr., Chief Clinical Officer at Sacramento Native American Health Center (SNAHC) shared insights on real-world implementation of AI tools in the safety net, focused on how SNAHC is using ambient scribe technology and decision support platforms to reduce clinician burden, improve patient engagement, and navigate language equity challenges. 


Listen in to learn more about how to implement AI solutions that address unique challenges and provide opportunities for FQHCs and safety net organizations to promote equitable access and improve patient outcomes.

Improving Care to Achieve Equity 

NOW AVAILABLE: CHW/P/R Medi-Cal Benefit Fact Sheet 

 

Since its launch in July 2022, the Medi-Cal CHW/P/R (Community Health Workers/Promotores/Representatives) Services Benefit recognizes CHW services as integral to Enhanced Care Management and Community Supports offered by managed care plans as part of CalAIM. However, it can be challenging to understand what scope of CHW/P/R services are covered, where services must be delivered, and how to get reimbursed by Medi-Cal for services. 

 

To create clarity, the Whole Child Equity Project, along with The Children’s Partnership, First 5 Center for Children’s Policy and the CHW/P/R Policy Coalition’s steering committee, have created a fact sheet to help community health providers better understand the Medi-Cal Community Health Worker Services Benefit. 

 

This Fact Sheet can be instrumental for CHW/P/R and community providers who need to better understand how to leverage and implement this benefit to build capacity in their organizations and communities.

RECORDING AVAILABLE: Redesigning Maternal Health: Integrating Doulas into Care Teams 


This webinar hosted by HealthLeads brought together maternal health experts to share three innovative case studies that demonstrate ways to overcome systemic barriers and build effective collaboration between doulas and healthcare organizations.

  • Showcase partnerships between hospital systems, clinical providers and doulas that improve care coordination and reducing disparities. 
  • Highlight strategies for building strong relationships to enhance patient experiences, reduce healthcare costs, and support doula workforce well-being.
