Safety Tools Refresher - Self-Check Using STAR
Stop - Pause for 1 to 2 seconds to focus our attention on the task at hand.
Think - Consider the action you're about to take.
Act - Concentrate and carry out the task.
Review - Check to make sure that the task was done correctly and that you got the correct result.

STOP is the most important step. It gives your brain a chance to catch up with what your hands are getting ready to do.

Clinical Documentation Tip of the Month: Bleeding Due to Therapeutic Anticoagulation

Any form of bleeding such as GI bleeding, epistaxis, hemoptysis or hematuria in a patient on Anticoagulation Therapy may be coded to D68.32.

The type of bleeding must be directly linked to the therapeutic anticoagulant in the Medical Record in order to capture this code, as well as the treatment rendered. The treatment may be as simple as holding the anticoagulation therapy, administering Vitamin K, transfusion of FFP, as well as addressing the source of the bleed.

Example: 73 y/o M with Hx : A fib on Coumadin presents with hematuria.
The documentation should link the Coumadin induced coagulopathy to
the Hematuria.
