CMS CROWNWeb/EQRS Upgrade Notice
Please Complete Your New HARP Security/Login Account Setup

CMS will continue upgrading CROWNWeb to EQRS (End Stage Renal Disease Quality Reporting System) soon. The first step in this upgrade path is to implement a new account/security login system called HARP, that will replace your existing EIDM CROWNWeb account. To make this process easier, CMS has migrated your existing CROWNWeb login to the new HARP system. 

We need your help to make sure all facility CROWNWeb users take the following actions to complete HARP account setup.  Failure to complete the following steps will result in loss of access to CROWNWeb when the next CROWNWeb software upgrade occurs.
Existing CROWNWeb/EQRS Users
Make Sure You Can Log Into CROWNWeb

This confirms that your account is active and working before attempting to complete HARP account setup. If you cannot log into CROWNWeb, contact the QualityNet Helpdesk to unlock/reset your account. You must be able to log into CROWNWeb in order to complete the following steps:

  1. Log into HARP: Go to the HARP login page and login with your CROWNWeb username and password.
  2. Set up HARP TFA: You will be asked to set up a two-factor authentication (TFA) device such as SMS text, phone number or other app.
  3. Complete your HARP Challenge Question: Select Login to HARP Profile to continue and complete your HARP Challenge Question. In the future, the QualityNet Helpdesk can help reset your HARP password only if you have completed your challenge question.
  4. Verify that your HARP email is Correct: Once logged into HARP, confirm that your email address is correct in your profile settings. Again, this is needed should you ever need to reset your account.
  5. Set up a second TFA device if possible (Optional): Once logged into HARP, you may wish to set up a second TFA device in case your primary TFA device is not available.
  6. HARP Account Setup is Complete – Now we Wait: If the above steps are successfully completed, you may log out of HARP and continue to use CROWNWeb as you always have. Your new HARP account is ready for the next CROWNWeb upgrade and will not be used until that time.
If you do not currently have a CROWNWeb account, but will need one, you may register for HARP as a new user by taking the following steps. Note that your HARP account will not allow CROWNWeb access until product upgrade in March.
1.      Go to the HARP Login
2.      Select “Sign Up” below the blue login button and follow the prompts.
