This year’s enrollment is completely online . You will log into the website below to upload the required documents and complete enrollment forms. You have until today Wednesday, June 30, 2021, at 11:59 pm.

Go to https://documentupload.yepsonline.org
Register for the site by filling in the requested information
Select the “Continue” button
Complete and electronically sign the Participant Enrollment Survey
Complete and electronically sign the I-9 Form
Complete and electronically sign the W-4 Form
Upload all listed documents
