Estimados Padres de Familia:

Como es de su conocimiento, todas las familias del Colegio Bolívar debieron diligenciar, el semestre pasado, el Formulario de Reporte de Condiciones Asociadas a Edad y Presencia de Comorbilidades de Riesgo COVID-19, tal como lo ordenaron los entes gubernamentales de salud y educación.
 
Es de suma importancia, para nosotros como institución educativa y para las entidades gubernamentales, tener actualizada esta información para contar con una caracterización veraz de nuestra población.

Si por alguna razón se presenta un cambio en el estado de salud de su hijo(a) o de un miembro de su núcleo familiar se debe de manera inmediata actualizar dicha información accediendo al siguiente enlace. Este formulario se debe diligenciar por cada hijo(a).

Les recordamos que para ingresar a diligenciar el formulario, deberán utilizar la clave y usuario de acceso que les fue enviada a cada familia a través de correo electrónico personalizado para ingresar al portal de la Elección del Modelo de Aprendizaje y Servicio de Transporte. Este formulario también está disponible en la sección Servicios en Línea de nuestra página web.

Dear Colegio Bolivar Parents,
 
As you know, all Colegio Bolivar families had to fill out the Report Form for Conditions Associated with Age and Presence of COVID-19 Risk Comorbidities last semester as ordered by the governmental bodies of health and education.
 
It is extremely important for us as an educational institution and for government entities to have this information updated, in order to have a reliable characterization of our population in terms of the breadth of the pandemic in our community.

If for any reason there is a change in the health status of your child or a member of your family, we invite you to immediately update the information by accessing the following link. This form must be completed for each child.

To fill out the form, you must use the password and access code that has been sent to each family by a personalized e-mail to enter the portal for the Choice of the Learning Model and Transportation Service. This form is also available in our webpage Online Services section.
