URGENT NOTICE RE: WALGREENS VACCINE CLINICS

Walgreens is now scheduling clinics to administer 1st dose to staff and others during the 2nd clinic.

Be on the lookout for a call or email from Walgreens if you have not already received your vaccination schedule.

Please note:

  1. Whoever started vaccinating will finish vaccinating either CVS, Walgreens or CDR
  2. The federal pharmacy program only allotted allocation of vaccine for 3 clinics at this point
CDR LAUNCHES NEW VACCINE WEBSITE

CDR has created the website, https://cdr-health.com/ltcvaccine/, to provide a central location for ALF’s and other long term care facilities to:
 
  • Access CDR Registration Form for download and completion
  • Update their information and required amount of vaccine
  • Access Consent Forms
  • Download a What to Expect Guide for Administrators regarding the CDR vaccination process onsite
  • Access information about the Pfizer Vaccine, VAERS program, and CDC information
  • FAQs as provided by ACHA
  • How to get in contact with CDR (last FAQ)
