COVID-19 Vaccine for Eligible Faculty & Staff
Dear Assumption Faculty and Staff,

Assumption University is collaborating with the Higher Education Consortium of Central Massachusetts (HECCMA) to secure COVID-19 vaccines for faculty and staff eligible to receive a vaccine under current Commonwealth guidelines. (See below for the latest eligibility guidelines.) The first dose will be administered sometime in March with the second dose to follow in April at a location to be determined in Worcester.

The University must request from the Commonwealth next week the number of vaccines it requires for eligible faculty and staff. If you would like to submit your name for consideration of the vaccine, please complete this form no later than Sunday, February 21 at 9 p.m. Health Services will review all submissions to determine eligibility. Completing the form does not guarantee that you will receive a vaccine as supplies may be limited. (Note that the form is not secure and you must first be logged into the Portal to access the form.)
Per the Commonwealth, those who are members of one or more of the below groups are eligible to receive a vaccine:
  • Aged 65 and older
  • Healthcare Workers and/or Police, Fire or EMS
  • Medical, Dental, or Nursing Student
  • Mental Health, Food Service, Custodial, Transportation and Retail Workers and Athletic Trainers.
  • Individuals with two or more Qualified Comorbidities (Cancer, Chronic Kidney Disease, COPD, Down Syndrome, Heart Conditions, Immunocompromised States, Obesity and Severe Obesity - BMI 30+, Pregnancy, Sickle Cell Disease, Smoking, Diabetes and Asthma)
  • Residents/Staff of Low-Income Housing or Affordable Senior Housing

While the University is not mandating the vaccine at this time, we wish to make this opportunity available to those who are eligible.

Sincerely,

Assumption University Health Services
